Analysis of the use of potentially inappropriate medication in elderly hospitalized in an Intensive Care Unit in a large hospital in Recife

Resumo: | Aim: To analyze the use of potentially inappropriate drugs prescribed to the elderly in Intensive Care Units. Methodology: Observational, descriptive, cross-sectional and retrospective study. The study was carried out by analyzing the electronic medical records of patients over 60 years of age, using potentially inappropriate drugs in the elderly with action on the central nervous system. Clinical and sociodemographic data were collected, as well as data regarding the use of these drugs (active pharmaceutical ingredient, drug class, treatment time, clinical indication and adverse reactions). Results: The most prescribed therapeutic classes were benzodiazepines, antipsychotics and hypnotics. Among the uses, 7.6% had no clinical justification, and the other justifications included medication reconciliation, hyperactive delirium, weaning from sedation and insomnia as the most frequent. Adverse reactions such as lowered level of consciousness, daytime sleepiness, constipation, mental confusion, and others have been observed. The results obtained are in agreement with the expected results regarding the possibility of contributing to the discussion on the current scenario of the use of potentially inappropriate drugs for the elderly in Intensive Care Units and possible perspectives and therapeutic alternatives. Conclusion: It is important to emphasize the importance of monitoring the use of these drugs when their use is unavoidable, as well as avoiding or reducing their prescription, associating other therapies with lower risk, in view of the potential harmful effects of inappropriate drugs. |

Metodología: Estudio observacional, descriptivo, transversal y retrospectivo. El estúdio se llevó a cabo mediante el análisis de historias clínicas electrónicas de pacientes mayores de 60 años, utilizando fármacos potencialmente inapropiados en ancianos con acción sobre el sistema nervioso central. Se recogieron datos clínicos y sociodemográficos, así como datos sobre el uso de estos fármacos (principio activo, clase, tiempo de tratamiento, indicación clínica y reacciones adversas). Resultados: Las clases terapéuticas más prescritas fueron las benzodiacepinas, los antipsicóticos y los hipnóticos. Entre los usos, el 7,6% no tuvo justificación clínica, y las demás justificaciones incluyeron la conciliación de medicamentos, el delirio hiperactivo, el destete de la sedación y el insomnio como los más frecuentes. Se han observado reacciones adversas como disminución del nivel de conciencia, somnolencia diurna, estreñimiento, confusión mental y otras. Los resultados obtenidos están de acuerdo con los resultados esperados en cuanto a la posibilidad de contribuir a la discusión sobre el escenario actual del uso de medicamentos potencialmente inapropiados para los ancianos en Unidades de Cuidados Intensivos y posibles perspectivas y alternativas terapéuticas. Conclusión: Es importante resaltar la importancia de monitorear el uso de estos medicamentos cuando su uso es inevitable, así como evitar o reducir su prescripción, asociando otras terapias con menor riesgo, ante los potenciales efectos nocivos de los medicamentos inapropiados.Objetivo: Analisar o uso de medicamentos potencialmente inapropriados prescritos a idosos em Unidades de Terapia Intensiva. Metodologia: Estudo observacional, descritivo, transversal e retrospectivo. Foi realizado por meio de análise de prontuários eletrônicos de pacientes com mais de 60 anos, em uso de medicamentos potencialmente inapropriados em idosos com ação no sistema nervoso central. Foram coletados dados clínicos e sóciodemográficos, bem como dados referentes ao uso destes medicamentos (princípio ativo, classe, tempo de tratamento, indicação clínica e reações adversas). Resultados: As classes terapêuticas mais prescritas foram os benzodiazepínicos, antipsicóticos e hipnóticos. Dentre os usos, 7,6% não tiveram justificativa clínica, e as demais justificativas incluíram conciliação medicamentosa, delirium hiperativo, desmame de sedação e insônia como as mais frequentes. Reações adversas como rebaixamento de nível de consciência, sonolência diurna, constipação, confusão mental, e outras foram observadas. Os resultados obtidos estão de acordo com os resultados esperados no que se refere à possibilidade de contribuição para discussão sobre o panorama atual do uso de medicamentos potencialmente inapropriados a idosos em Unidades de Terapia Intensiva e possíveis perspectivas e alternativas terapêuticas.
