Applicability of the Medical Iatrogenic Scale: pilot test

Resumo: The iatrogenesis, derived from the communication process, acts on the subjectivity of the patient, and may occur within the scope of the clinical encounter. This fact demonstrates the importance of a meeting with a doctor who understands its iatrogenic potential. This study aimed to evaluate the applicability and effectiveness of the Medical Iatrogenic Scale (EIM-20) in the context of the physician-patient relationship. This is pilot research for the construction and validation of a psychometric scale. This study was conducted in four stages. The first consisted of choosing and defining the dimensions that made up the scale, observed through an extensive literature review on iatrogenics in medicine. In the second, the items were submitted to theoretical analysis with five highly specialized doctors (called judges), to identify coherence between the definitions (dimensions) and the behaviors (items) that empirically represent the phenomenon. The third stage consisted of a validation phase of the construct, where the scale was applied to fifty doctors who make up the staff of professors and preceptors of the medical course at PUC Goiás. In the fourth and last, statistical tests were applied that helped in the validation of the scale. The pilot test proved to be very viable by presenting a Cronbach's alpha that allows the continuation of the research in a larger sample of the medical population. Therefore, the EIM-20 is promising to be applied on a large scale in the medical population, given that the results presented confirm the reliability of the psychometric assessment of the scale.

Este hecho demuestra la importancia de una consulta con un médico que comprenda su potencial iatrogénico. Este estudio tuvo como objetivo evaluar la aplicabilidad y efectividad de la Escala Médica Iatrogénica (EIM-20) en el contexto de la relación médico-paciente. Esta es una investigación piloto para la construcción y validación de una escala psicométrica. Este estudio se llevó a cabo en cuatro etapas. La primera consistió en elegir y definir las dimensiones que componían la escala, observadas a través de una extensa revisión bibliográfica sobre iatrogenia en medicina. En el segundo los ítems fueron sometidos a análisis teórico con cinco médicos altamente especializados (llamados jueces), con el fin de identificar coherencia entre las definiciones (dimensiones) y los comportamientos (ítems) que representan empíricamente el fenómeno. La tercera etapa consistió en una fase de validación del constructo, donde la escala fue aplicada a 50 médicos que integran el cuadro de profesores y preceptores del curso de medicina de la PUC Goiás. En el cuarto y último se aplicaron pruebas estadísticas que ayudaron en la validación de la escala. La prueba piloto demostró ser muy viable al presentar un alfa de Cronbach que permite continuar con la investigación en una muestra mayor de población médica. A terceira etapa consistiu em uma fase de validação do construto, onde a escala foi aplicada a 50 médicas (os) que compõem o quadro de docentes e preceptores do curso de medicina da PUC Goiás. Na quarta e última foram aplicados testes estatísticos que auxiliaram na validação da escala. O teste piloto se mostrou muito viável ao apresentar um alfa de Cronbach que permite a continuação da pesquisa em uma amostra maior da população médica.
