Variability of Heart Rate as a biomarker of stress: integrative review

Resumo: Objective: to identify the applicability of Heart Rate Variability (HRV) as a biomarker of stress through an integrative literature review. Method: for this purpose, a search was conducted in the Web of Science ® and Medical Literature Analysis and Retrieval System Online (MEDLINE) databases. The descriptors Heart Rate Variability, HRV, Autonomous Nervous System (ANS), RR Intervals and stress were combined in different ways in order to allow greater capture of studies. Inclusion criteria were studies involving humans, association between HRV and stress or ANS, and HRV analysis using linear and non-linear measurements. Eighteen publications were identified that met the inclusion and exclusion criteria. A descriptive analysis of these works was carried out. Results: the results showed that HRV characteristics were altered in response to stress. HRV indices obtained by linear methods (time domain and frequency domain) and non-linear methods were cited in some studies as a clinical diagnostic tool to investigate cardiovascular interactions, evaluate physiological mechanisms and trace their changes in different physiological states. Conclusion: it is concluded that HRV can be used to evaluate stress because it reflects cardiac activity and ANS activities. It was observed that the different methods used to induce stress were able to stimulate an ANS response through the changes in HRV parameters.

Método: para ello, se realizó una búsqueda en las bases de datos Web of Science® y Medical Literature Analysis and Retrieval System Online (MEDLINE). Los descriptores Variabilidad de la frecuencia cardíaca, VFC, Sistema Nervioso Autónomo (SNA), Intervalos RR y estrés se combinaron de diferentes formas para permitir una mayor captación de los estudios. Los criterios de inclusión fueron estudios con seres humanos, asociación entre VFC y estrés o ANS y análisis de VFC utilizando medidas lineales y no lineales. Se identificaron dieciocho publicaciones que cumplieron con los criterios de inclusión y exclusión. Se realizó un análisis descriptivo de estos trabajos. Resultados: los resultados mostraron que las características de la VFC se alteraron en respuesta al estrés. Los índices de VFC obtenidos por métodos lineales (dominio de tiempo y dominio de frecuencia) y los métodos no lineales se han citado en algunos estudios como una herramienta de diagnóstico clínico para investigar interacciones cardiovasculares, evaluar mecanismos fisiológicos y rastrear sus cambios en diferentes estados fisiológicos. Conclusión: se concluye que la VFC se puede utilizar para evaluar el estrés, ya que refleja la actividad cardíaca y las actividades del SNA. Se observó que los diferentes métodos utilizados para inducir el estrés fueron capaces de estimular una respuesta del SNA a través de cambios en los parámetros de la VFC.Objetivo: identificar a aplicabilidade da Variabilidade da Frequência Cardíaca (VFC) como biomarcador do estresse por meio de uma revisão integrativa da literatura. Os descritores Variabilidade da Frequência Cardíaca, VFC, Sistema Nervoso Autônomo (SNA), Intervalos RR e estresse foram combinados de diferentes formas a fim de possibilitar maior captação de estudos. Os critérios de inclusão foram estudos envolvendo seres humanos, associação entre VFC e estresse ou SNA e análise da VFC utilizando medidas lineares e não lineares. Foram identificadas 18 publicações que atenderam aos critérios de inclusão e exclusão. Foi realizada uma análise descritiva desses trabalhos. Resultados: os resultados mostraram que as características da VFC foram alteradas em resposta ao estresse. Os índices da VFC obtidos por métodos lineares (domínio do tempo e domínio da frequência) e os métodos não lineares foram citados em alguns estudos como ferramenta de diagnóstico clínico para investigar as interações cardiovasculares, avaliar os mecanismos fisiológicos e rastrear suas mudanças em diferentes estados fisiológicos.  Conclusão: conclui-se que a VFC pode ser usada para avaliar o estresse, pois ela reflete a atividade cardíaca e as atividades do SNA.
