Safe surgery in cheiloplasty and palatoplasty procedures in patients with craniofacial anomalies

Resumo: Objective: To evaluate the outcome of compliance with the safe surgery protocol in cheiloplasty and palatoplasty procedures. Methodology: This is a cross-sectional, observational study carried out in a surgical center of a public teaching hospital in southern Brazil. The sample consisted of all surgeries performed between May and October 2021. The following study variables were evaluated: sex, age, type of surgery, type of anesthesia, surgery time, number of people in the operating room, completion of the checklist of safe surgery, completion of checklist steps and identified risk factors. Data analysis was performed using descriptive statistics. Results: 27 (100%) surgeries were performed during the study period, 59.3% of which were cheiloplasty and 40.7% were palatoplasty. There was a predominance of male patients (63%) in both procedures. Lack of beds (64%) and inadequate clinical status (36%) were the main reasons for surgical cancellation. There was a predominance of size I surgeries (77.8%) and it was observed that 74% of the surgeries had six to ten professionals in the room. The safe surgery protocol was performed in most of the surgical procedures investigated (74%), however, half of this amount was performed incompletely (37%). The greatest absence of checking occurred in the Time Out (59.2%). The study identified irregular completion of the surgical consent form in only 18.5% of the sample. Conclusion: even with failures in the execution of the safe surgery checklist, no adverse events were observed in the surgeries performed.

Metodología: Se trata de un estudio transversal, observacional, realizado en un centro quirúrgico de un hospital público de enseñanza del sur de Brasil. La muestra estuvo conformada por todas las cirugías realizadas entre mayo y octubre de 2021. Se evaluaron las siguientes variables de estudio: sexo, edad, tipo de cirugía, tipo de anestesia, tiempo de cirugía, número de personas en quirófano, cumplimentación del checklist de cirugía segura, finalización de los pasos de la lista de verificación y factores de riesgo identificados. El análisis de los datos se realizó mediante estadística descriptiva. Resultados: se realizaron 27 (100%) cirugías durante el período de estudio, de las cuales el 59,3% fueron queiloplastia y el 40,7% palatoplastia. Hubo predominio de pacientes masculinos (63%) en ambos procedimientos. La falta de camas (64%) y el inadecuado estado clínico (36%) fueron los principales motivos de cancelación quirúrgica. Hubo predominio de cirugías de tamaño I (77,8%) y se observó que el 74% de las cirugías tenían de seis a diez profesionales en la sala. El protocolo de cirugía segura se realizó en la mayoría de los procedimientos quirúrgicos investigados (74%), sin embargo, la mitad de esta cantidad se realizó de forma incompleta (37%). La mayor ausencia de verificación ocurrió en el Tiempo Fuera (59,2%). El estudio identificó cumplimentación irregular del formulario de consentimiento quirúrgico en sólo el 18,5% de la muestra. Conclusión: aún con fallas en la ejecución del checklist de cirugía segura, no se observaron eventos adversos en las cirugías realizadas. Objetivo: Avaliar o desfecho do cumprimento do protocolo de cirurgia segura em procedimentos de queiloplastia e palatoplastia. Metodologia: Trata-se de estudo transversal, observacional realizado em um centro cirúrgico de hospital público de ensino do sul do Brasil. A amostra foi composta pela totalidade de cirurgias realizadas entre maio e outubro de 2021. Foram avaliadas as seguintes variáveis de estudo: sexo, idade, tipo de cirurgia, tipo de anestesia, tempo de cirurgia, número de pessoas na sala operatória, realização do checklist de cirurgia segura, completude da execução das etapas do checklist e fatores de risco identificados. A análise dos dados foi realizada por estatística descritiva. Resultados: Foram realizadas 27(100%) cirurgias no período de estudo, sendo 59,3% de queiloplastias e 40,7% de palatoplastia. Houve predominância de pacientes do sexo masculino (63%) em ambos os procedimentos. A falta de leito (64%) e o quadro clínico inadequado (36%) foram os principais motivos para o cancelamento cirúrgico. Houve predominância de cirurgias de porte I (77,8%) e observou-se que 74% das cirurgias apresentavam de seis a dez profissionais em sala. O protocolo de cirurgia segura foi realizado na maioria dos procedimentos cirúrgicos investigados (74%), no entanto, a metade desse montante foi realizada de forma incompleta (37%). A maior ausência de checagem ocorreu no Time Out (59,2%). O estudo identificou irregularidade no preenchimento do termo de consentimento cirúrgico em 18,5 % da amostra.
