Periodontitis and Metabolic syndrome: differences between criteria of diagnosis and sex

Resumo: Introduction: Periodontitis is characterized by tissue changes such as gingival inflammation, destruction of the periodontal ligament and the alveolar bone. Metabolic syndrome (MS) is defined by a set of metabolic changes (dyslipidemia, arterial hypertension, glucose intolerance, central obesity and insulin resistance), which commonly manifest as comorbidities. Objective: To compare different criteria for diagnosing periodontitis, as well as the criteria for diagnosing MS and to identify differences between them according to sex. Method: Cross-sectional study conducted with 90 users of the Family Health Unit (USF) Vale do Cambonas in Salvador-Ba, developed with the application of a questionnaire, physical / anthropometric, dental, medical records and laboratory exam reports. Results: Good agreement was verified between the criteria for the diagnosis of MS (Kappa = 0.8230) and average agreement comparing three different periodontitis diagnostic criteria proposed. Men had worse periodontic conditions than women. These showed worse metabolic rates. As for the number of teeth, there was a trend towards an association between tooth loss and diabetes. Conclusion: The divergence observed between the diagnostic criteria of periodontitis corroborates the difficulty in standardizing this diagnosis, making it difficult to compare scientific findings and the differences found between genders related to the diagnosis of MS. Periodontal parameters and clinical and laboratory characteristics proved to be relevant, requiring further studies to deepen them.

El síndrome metabólico (SM) se define por un conjunto de cambios metabólicos (dislipidemia, hipertensión arterial, intolerancia a la glucosa, obesidad central y resistencia a la insulina), que comúnmente se manifiestan como comorbilidades. Objetivo: comparar diferentes criterios para el diagnóstico de periodontitis, así como los criterios para el diagnóstico de SM e identificar diferencias entre ellos según el sexo. Método: Estudio transversal realizado con 90 usuarios de la Unidad de Salud de la Familia (USF) Vale do Cambonas en Salvador-Ba, desarrollado con la aplicación de un cuestionario, registros físicos / antropométricos, odontológicos, médicos e informes de exámenes de laboratorio. Resultados: Se verificó buena concordancia entre los criterios para el diagnóstico de SM (Kappa = 0,8230) y concordancia promedio comparando três diferentes criterios diagnósticos de periodontitis propuestos. Los hombres tenían peores condiciones de periodoncia que las mujeres. Estos mostraron peores tasas metabólicas. En cuanto al número de dientes, hubo una tendencia hacia una asociación entre la pérdida de dientes y la diabetes. Conclusión: La divergencia observada entre los criterios diagnósticos de periodontitis corrobora la dificultad para estandarizar este diagnóstico, dificultando la comparación de hallazgos científicos y las diferencias encontradas entre géneros relacionadas con el diagnóstico de SM. Objetivo: Comparar critérios distintos para diagnóstico de periodontite, assim como os critérios de diagnósticos da SM e identificar diferenças entre os mesmos de acordo com o sexo. Método: Estudo transversal realizado com 90 usuários da Unidade de Saúde da Família (USF) Vale do Cambonas em Salvador-Ba, desenvolvido com aplicação de questionário, avaliações física/antropométrica, odontológica, de prontuários e laudos de exames laboratoriais. Resultados: Verificada boa concordância entre critérios de diagnóstico da SM (Kappa=0,8230) e concordância média comparando três diferentes critérios de diagnóstico da periodontite propostos. Os homens apresentaram condições periodontias piores do que as mulheres. Já estas apresentaram piores índices metabólicos. Quanto à quantidade de dentes, observou-se uma tendência de associação ente perda dentária e diabetes. Conclusão: A divergência observada entre os critérios diagnóstico da periodontite corrobora a dificuldade de padronização deste diagnóstico, dificultando a comparabilidade entre os achados científicos e as diferenças encontradas entre os gêneros relacionadas ao diagnóstico da SM.
