Front-of-pack food labeling in South America: A scoping review

Resumo: The aim of the study was to map and synthesize the scientific production of knowledge regarding front-of-package food labeling in South American countries. A scoping review was conducted in 2023, based on the Joanna Briggs Institute guide, using a search strategy across five electronic databases: LILACS, SciELO, PubMed, Web of Science, and Scopus, with no restrictions on language or year of publication. The analysis process was carried out through blinded review by two reviewers: 1) article selection using the Rayyan platform, 2) data extraction using a standardized form, 3) PRISMA flowchart and checklist, 4) frequency calculations, and organization of findings. The search strategy identified 2,711 records, of which 94 articles met the inclusion criteria. There was a prevalence of studies from Uruguay and from the year 2020; 61% of the studies were cross-sectional observational, and 57% had a local scope. Study outcomes addressed the nutritional quality and composition of foods, the food industry, types of labeling, and consumer food choices. The synthesis revealed heterogeneity in results regarding the effectiveness of labeling types and the nutritional quality of packaged foods due to different populations studied. The mapping allowed the identification of resistance from the food industry to policy implementation, even in the face of evidence of improved consumer food choices, regardless of the type of front-of-package labeling used. It is suggested that new studies investigate different labeling methodologies more robustly across different countries.

Se llevó a cabo una revisión de alcance en 2023, basada en la guía del Instituto Joanna Briggs, utilizando estrategia de búsqueda en cinco bases de datos electrónicas: LILACS, SciELO, PubMed, Web of Science y Scopus, sin restricciones de idioma o año de publicación. El proceso de análisis se realizo mediante revisión ciega por dos revisores: 1) selección de artículos en plataforma Rayyan, 2) extracción de datos con formulario estandarizado, 3) diagrama de flujo y lista de verificación PRISMA, 4) cálculos de frecuencias y organización de hallazgos. La estrategia de búsqueda identificó 2.711 registros, 94 artículos cumplieron con los criterios de inclusión. Hubo predominio de estudios de Uruguay y del año 2020; 61% eran observacionales transversales y 57% tenían alcance local. Los resultados abordaron la calidad nutricional y composición de los alimentos, la industria alimentaria, tipos de etiquetado y la elección de alimentos del consumidor. La síntesis reveló una heterogeneidad en los resultados en cuanto a la efectividad de los tipos de etiquetado y la calidad nutricional de los alimentos envasados debido a las diferentes poblaciones estudiadas. El mapeo permitió identificar la resistencia de la industria alimentaria a la implementación de políticas, incluso ante evidencias de mejoras en la elección alimentaria del consumidor, independientemente del tipo de etiquetado frontal utilizado. Se sugiere que nuevos estudios investiguen diferentes metodologías de etiquetado de manera más robusta en diferentes países.O objetivo do estudo foi mapear e sintetizar a produção científica do conhecimento sobre a rotulagem frontal de embalagens de alimentos de países sul-americanos. Foi realizada uma revisão de escopo em 2023, baseada no guia do Instituto Joanna Briggs, a partir de uma estratégia de busca em cinco bases de dados eletrônicas: LILACS, SciELO, PubMed, Web of Science e Scopus, sem restrições quanto ao idioma ou ano de publicação. O processo de análise foi realizado em cegamento por dois revisores: 1) seleção dos artigos com o uso da plataforma Rayyan, 2) extração dos dados por formulário padrão, 3) fluxograma e checklist PRISMA, 4) cálculos de frequências e organização dos achados. A estratégia de busca identificou 2.711 registros, dos quais 94 artigos atenderam os critérios de inclusão. Houve o predomínio de estudos do Uruguai e em 2020; 61% dos estudos eram observacionais transversais e 57% de abrangência local. Os desfechos dos estudos abordaram a qualidade nutricional e a composição dos alimentos; a indústria alimentícia; os tipos de rotulagens e a escolha alimentar do consumidor. A síntese trouxe uma heterogeneidade dos resultados quanto a efetividade dos tipos de rotulagem e a qualidade nutricional dos alimentos embalados devido aos diferentes tipos de populações estudadas. O mapeamento possibilitou identificar a resistência da indústria alimentícia quanto a implementação da política mesmo diante de evidências da melhora da escolha alimentar do consumidor, independente do tipo de rotulagem frontal utilizada.
