Netherton Syndrome: diagnosis, forms of treatment and the role of pharmaceutical care for health in this pathology

Resumo: Netherton Syndrome (NS) is a very rare and severe dermatosis, characterized by reddened skin with abundant desquamation, generating rapid skin regeneration and causing fluid loss. Diagnosis is challenging and can often be confused with other comorbidities. NS has no cure, only treatment. The pharmaceutical care of this disease helps to improve the patient's quality of life, as well as better adaptation to treatment. This work aims to survey, based on the current literature, the characteristics of SN, to facilitate the diagnosis; demonstrate its various forms of treatment; analyze the role of the pharmacist to improve the quality of life of the carrier. For this investigation, a bibliographic research was adopted, based on a literature review of scientific articles available in the electronic database of the Scientic Electronic Library Online (SciELO) and Medical Literature Analysis And Retrieval System Online (MEDLINE). In the reviewed bibliographies it was possible to find that NS is caused by genetic mutation, it has a complex diagnosis, and bullous ichthyosis, atopic dermatitis, among others, can be confused. Some of the treatments are used emollients, reducing transepidermal water loss, dupilumab is also used, a biological medication of human monoclonal IgG4 antibody, improving the condition of the skin, as well as pharmaceutical care in this scenario brings great improvements in adherence to treatment. It is noticed that there is still a lot of study to be carried out and to be able to further improve the well-being of these individuals, as well as more research on the syndrome in order to make a quick diagnosis and effective treatments possible.

El diagnóstico es un desafío y, a menudo, se puede confundir con otras comorbilidades. El SN no tiene cura, solo tratamiento. La atención farmacéutica de esta enfermedad ayuda a mejorar la calidad de vida del paciente, así como a una mejor adaptación al tratamiento. Este trabajo tiene como objetivo relevar, con base en la literatura actual, las características de la NS, para facilitar el diagnóstico; demostrar sus diversas formas de tratamiento; Analizar el papel del farmacéutico para mejorar la calidad de vida del paciente. Para esta investigación se adoptó una investigación bibliográfica, basada en una revisión de la literatura de artículos científicos disponibles en la base de datos electrónica de la Biblioteca Electrónica Científica en Línea (SciELO) y el Sistema de Recuperación y Análisis de Literatura Médica en Línea (MEDLINE). En las bibliografías revisadas se pudo encontrar que el SN es causado por una mutación genética, tiene un diagnóstico complejo y puede confundirse con ictiosis ampollosa, dermatitis atópica, entre otras. En algunos de los tratamientos se utilizan emolientes, reduciendo la pérdida de agua transepidérmica, también se utiliza dupilumab, un medicamento biológico de anticuerpo monoclonal IgG4 humano, mejorando el estado de la piel, así como la atención farmacéutica en este escenario trae grandes mejoras en la adherencia al tratamiento. Se advierte que aún quedan muchos estudios por realizar y para poder seguir mejorando el bienestar de estos individuos, así como más investigaciones sobre el síndrome con el fin de hacer posible un diagnóstico rápido y tratamientos efectivos.A Síndrome de Netherton (SN) é uma dermatose bastante rara e grave, tem como características pele avermelhada com abundante descamação, gerando regeneração rápida da pele e provocando perda de líquidos. O diagnóstico é desafiador, geralmente pode ser confundida com outras comorbidades. A SN não possui cura, apenas tratamento. O cuidado farmacêutico deste agravo ajuda na melhoria da qualidade de vida do paciente, como também melhor adaptação ao tratamento. Este trabalho tem como objetivo levantar com base na literatura atual as características da SN, facilitar o diagnóstico; demostrar suas diversas formas de tratamento; analisar o papel do farmacêutico para melhorar a qualidade de vida do portador. Para essa investigação adotou-se uma pesquisa bibliográfica, fundamentada em revisão de literatura de artigos científicos disponibilizados no banco de dados eletrônico da Scientic Eletronic Library Online (SciELO) e Medical Literature Analysis And Retrieval System Online (MEDLINE). Nas bibliografias revisadas foi possível levantar que a SN é causada por mutação genética, possui diagnóstico complexo, podendo ser confundida ictiose bolhosa, dermatite atópica, entre outros. Alguns dos tratamentos é utilizado emolientes, diminuindo a perda transepidérmica de água, também se utiliza o dupilumabe, medicação biológica de anticorpo monoclonal humano IgG4, melhorando a condição da pele, bem como o cuidado farmacêutico neste cenário traz grandes melhorias na adesão ao tratamento.
