Morphological characteristics and clinical and surgical implications of the supreme nasal concha: an integrative review

Resumo: Introduction: On the lateral wall of each nasal cavity, there are three conchas named according to their position: superior, middle and inferior. Eventually, there may be a fourth concha (of Santorini), known as the supreme concha. However, its specific characteristics are scarce in the literature, being superficially addressed in scientific studies and textbooks. Objective: To describe the morphological aspects of the supreme concha and analyze its clinical and surgical importance. Methodology: Integrative literature review with searches in PubMed, Scopus, Web of Science, Embase and MEDLINE databases. As a search tool, the descriptors “supreme concha nasal”, “supreme turbinate” and “concha Santorini” were used, as well as the Boolean operator “or”. Eligibility criteria consisted of original articles without language and publication period restrictions. Results and Discussion: Among 21 non-duplicated articles, six were selected. The supreme concha is found more bilaterally and its prevalence varies from 10% to 77% (average of 42.3%), being predominant in males and having a higher frequency in the second trimester of intrauterine life, in addition to having a similar size to the superior concha in most cases. The knowledge of the supreme concha is important for many endoscopic surgical procedures of the sinuses and skull base. Conclusion: The supreme concha is more common than previously thought. Imaging studies in computed tomography, mainly with 3D reconstruction, revealed a higher prevalence than anatomical studies in cadavers. It is inferred that the Santorini concha may not be an anatomical variation in some populations.

Objetivo: Describir los aspectos morfológicos del cornete supremo y analizar su importancia clínica y quirúrgica. Metodología: Revisión integrativa de la literatura con búsquedas en las bases de datos PubMed, Scopus, Web of Science, Embase y MEDLINE. Como herramienta de búsqueda se utilizaron los descriptores “concha nasal suprema”, “concha nasal suprema” y “concha Santorini”, así como el operador booleano “o”. Los criterios de elegibilidad consistieron en artículos originales sin restricciones de idioma y período de publicación. Resultados y Discusión: Entre 21 artículos no duplicados, seis fueron seleccionados. A concha suprema é mais encontrada bilateralmente e sua prevalência varia de 10% a 77% (média de 42,3%), sendo predominante no sexo masculino e tendo maior frequência no segundo trimestre de vida intrauterina, além de ter tamanho semelhante a concha superior en la mayoría de los casos. Aunque es una variación anatómica, el conocimiento de la concha suprema es importante para muchos procedimientos quirúrgicos endoscópicos de los senos paranasales y la base del cráneo. Conclusión: El caracol supremo es más común de lo que se pensaba. Los estudios de imagen en tomografía computarizada, principalmente con reconstrucción 3D, revelaron una mayor prevalencia que los estudios anatómicos en cadáveres. Embora seja uma variação anatômica, o conhecimento da concha suprema é importante para vários procedimentos cirúrgicos endoscópicos dos seios da face e da base do crânio. Conclusão: A concha suprema é mais comum do que se pensava. Estudos imagiológicos em tomografias computadorizadas, principalmente com reconstrução 3D, revelaram maior prevalência do que estudos anatômicos em cadáveres.
