Screening and comparison of critical patient mobility indicators at a university hospital

Resumo: Objective: To compare the mobility of patients at admission and discharge, as well as between the General ICU and Covid ICU sectors of a University Hospital in Sergipe, through the analysis of care indicators. Methods: This is a longitudinal observational study developed in 2020. Mobility indicators were obtained from the result of the Mobility Scale in the ICU (IMS). Statistical analysis was performed using the SPSS(11.5) program. To compare the indicators on admission and discharge from the ICUs, the Chi-square and Man Whitney tests were used. Results: At admission, 78.9% in the Covid ICU and 58% in the General had zero IMS. There was an improvement at the time of discharge of 14.6% in Covid and 37.9% in General. As for the time to first standing, the average number of days was 9.8(±8.4) in Covid and 11.2(±9.1) in General. For walking, it was seen that 32% and 23% of the Covid and General ICUs, respectively, walked. There was a significant difference between the General and Covid ICUs only for IMS zero on admission (p=0.021). At discharge, there was no significant difference between the indicators evaluated. In both units no worsening of IMS was observed. Conclusion: It was observed that most patients were admitted to bed, with high rates of zero IMS, with a significant difference between the ICUs. However, at discharge, there was an improvement in the indicators of IMS, with no significant difference between the units, but with still reduced rates, showing an important impairment in the mobility of these patients at discharge from the ICU.

Métodos: Se trata de un estudio observacional longitudinal desarrollado en 2020. Los indicadores de movilidad se obtuvieron a partir del resultado de la Escala de Movilidad en UCI (IMS). El análisis estadístico se realizó mediante el programa SPSS(11.5). Para comparar los indicadores de ingreso y egreso de las UCI se utilizaron las pruebas de Chi-cuadrado y Man Whitney. Resultados: Al ingreso, el 78,9% en la UCI Covid y el 58% en la General tenían cero SGI. Hubo una mejoría en el momento del alta del 14,6% en Covid y del 37,9% en General. En cuanto al tiempo hasta la primera puesta, el promedio de días fue de 9,8 (± 8,4) en Covid y de 11,2 (± 9,1) en General. Para caminar, se observó que el 32% y el 23% de las UCI Covid y General, respectivamente, caminaron. Hubo una diferencia significativa entre las UCI General y Covid solo para IMS cero al ingreso (p = 0.021). Al alta, no hubo diferencia significativa entre los indicadores evaluados. En ambas unidades no se observó ningún empeoramiento de la IMS. Conclusión: Se observó que la mayoría de los pacientes ingresaron en cama, con altas tasas de IMS cero, con una diferencia significativa entre las UCI. Sin embargo, al alta, hubo una mejora en los indicadores de IMS, sin diferencia significativa entre las unidades, pero con tasas aún reducidas, mostrando un importante deterioro en la movilidad de estos pacientes al alta de la UCI.Objetivo: Comparar a mobilidade dos pacientes no momento de internação e alta, bem como entre os setores UTI Geral e UTI Covid de um Hospital Universitário de Sergipe, através da análise dos indicadores assistenciais. Métodos: Trata-se de um estudo de caráter longitudinal do tipo observacional desenvolvido no ano de 2020. Os indicadores de Mobilidade foram obtidos através do resultado da escala de Mobilidade na UTI (IMS). A análise estatística foi realizada através do programa SPSS (11.5). Para comparar os indicadores na admissão e alta das UTIs foi utilizado teste Qui-quadrado e Man Whitney. Resultados: Na admissão,78,9% na UTI Covid  e  58% na Geral  possuíam IMS zero. Foi verificada melhora no momento da alta de 14,6% na Covid e 37,9% na Geral. Quanto ao tempo para primeiro ortostatismo a média de dias, foi de 9,8(±8,4) na Covid e 11,2(±9,1) na Geral. Para deambulação foi visto que 32% e 23% das UTI Covid e Geral, respectivamente, deambularam. Houve diferença significativa entre as UTI Geral e Covid apenas para IMS zero na admissão (p=0,021). Já na alta não houve diferença significativa entre os indicadores avaliados. Em ambas as unidades não foi observado piora de IMS. Conclusão: Foi observado que a maioria dos pacientes foram admitidos restritos ao leito, com altas taxas de IMS zero, havendo diferença significativa entre as UTIs.
