Methodological differences between gestational diabetes mellitus and prematurity: a systematic review

Resumo: Objectives: To assess methodological differences between studies that analyzed the association between gestational diabetes mellitus and prematurity. Methods: The selection of articles was carried out between October 2020 and January 2021 in the PubMed, Virtual Health Library (Lilacs/Bireme/BVS Brasil), Embase and Scopus databases. The following descriptors were used: “Gestational Diabetes Mellitus” and “Prematurity”, which were modified according to the indexing rules of each database. There was no restriction on publication period and language. Twelve studies were selected, being two cross-sectional, five cohorts, four control cases and one clinical trial. Results: Most studies used the National Diabetes Data Group criteria for the diagnosis of gestational diabetes mellitus. Of the selected studies, six observed an association between gestational diabetes mellitus and prematurity. Conclusion: In addition to the impact of the multifactorial causality of prematurity, studies that did not find an association between gestational diabetes mellitus and prematurity showed methodological differences, especially regarding the definition, screening and diagnosis of gestational diabetes mellitus, which may lead to different results.

Métodos: La selección de artículos se realizó entre octubre de 2020 y enero de 2021 en las bases de datos PubMed, Virtual Health Library (Lilacs / Bireme / BVS Brasil), Embase y Scopus. Se utilizaron los siguientes descriptores: “Diabetes Mellitus gestacional” y “Prematuridad”, los cuales fueron modificados de acuerdo a las reglas de indexación de cada base de datos. No hubo restricciones sobre el período de publicación y el idioma. Se seleccionaron doce estudios, siendo dos transversales, cinco cohortes, cuatro casos control y un ensayo clínico. Resultados: La mayoría de los estudios utilizaron los criterios del National Diabetes Data Group para el diagnóstico de diabetes mellitus gestacional. De los estudios seleccionados, seis observaron una asociación entre la diabetes mellitus gestacional y la prematuridad. Conclusión: Además del impacto de la causalidad multifactorial de la prematuridad, los estudios que no encontraron asociación entre diabetes mellitus gestacional y prematuridad mostraron diferencias metodológicas, especialmente en cuanto a la definición, cribado y diagnóstico de diabetes mellitus gestacional, que pueden conducir a resultados diferentes.Objetivos: Avaliar as divergências metodológicas entre os estudos que analisaram a associação entre o diabetes mellitus gestacional e a prematuridade. Métodos: A seleção dos artigos foi realizada entre outubro de 2020 e janeiro de 2021 nas bases de dados PubMed, Biblioteca Virtual em Saúde (Lilacs/Bireme/BVS Brasil), Embase e Scopus. Foram utilizados os seguintes descritores: “Gestational Diabetes Mellitus” and “Prematurity” que foram modificados de acordo com as regras de indexação de cada base de dados. Não houve restrição no período de publicação e idioma. Foram selecionados 12 estudos, sendo dois transversais, cinco coortes, quatro casos-controle e um ensaio clínico. Resultados: A maioria dos estudos utilizou o critério do National Diabetes Data Group para diagnóstico do diabetes mellitus gestacional. Dos estudos selecionados, seis observaram associação entre diabetes mellitus gestacional e a prematuridade.
