Prevalence of changes in maxillary sinus through digital panoramic radiographs of the Tiradentes University

Resumo: | The maxillary sinuses, an anatomical structure viewed radiographically by a wide radiolucent area in the posterior region of the maxilla and are the largest of the paranasal cavities and can be located on digital panoramic radiographs. In this area, extensions, intrasinusal septum and some pathological changes can be found. This research aims to investigate and determine the prevalence of the main types of anatomical and pathological changes in maxillary sinuses with their exact locations through digital panoramic radiographs from the image bank of the Radiology Clinic of the Tiradentes University (UNIT). The sample consisted of 878 exams, totaling 1756 maxillary sinuses. To assess the association between variables, the non-parametric Chi-square association test was used, with a level of statistical significance at p <0.05. To verify the degree of agreement between the examiners, the Kappa statistical test was used (p <0.001). The data obtained showed that there was a predominante of females (66.2%). A greater number of changes was observed among individuals aged 46 to 55 years (84.78%). Regarding anatomical changes, the intrasinusal septum (69.2%) were more frequent and, as pathological changes, the mucus retention cyst (1.17%). There was a statistical difference between the presence of sinus changes in relation to the age group (p <0.0001). There was a higher prevalence of sinus alterations in women, being statistically significant (<0.0001). This study contributed to the knowledge about the anatomical structure of the studied population, where digital panoramic radiography is an important tool for the diagnosis of changes in the maxillomandibular region. |

Los senos maxilares, una estructura anatómica visualizada radiográficamente por una amplia área radiolúcida en la región posterior del maxilar, se consideran la mayor de las cavidades paranasales y pueden localizarse en radiografías panorámicas digitales. En esta zona se pueden encontrar extensiones, septos intrasinusales y algunas alteraciones patológicas. Esta investigación tiene como objetivo investigar y determinar la prevalencia de los principales tipos de alteraciones anatómicas y patológicas en los senos maxilares con sus ubicaciones exactas a través de radiografías panorámicas digitales del banco de imágenes de la Clínica de Radiologia da Universidade Tiradentes (UNIT). La muestra estuvo constituida por 878 exámenes, totalizando 1756 senos maxilares. Para evaluar la asociación entre las variables se utilizó la prueba de asociación no paramétrica chi-cuadrado, con un nivel de significancia estadística de p<0,05. Para verificar el grado de concordancia entre los examinadores se utilizó la prueba estadística Kappa (p<0,001). Los datos obtenidos mostraron que hubo predominio del sexo femenino (66,2%). Se observó mayor número de alteraciones entre los individuos de 46 a 55 años (84,78%). En cuanto a las alteraciones anatómicas, fueron más frecuentes los septos intrasinusales (69,2%) y, como alteraciones patológicas, el quiste de retención mucoso (1,17%). Se observó diferencia estadística entre la presencia de alteraciones de los senos paranasales en relación al grupo de edad (p<0,0001). Hubo una mayor prevalencia de alteraciones de los senos paranasales en mujeres, lo que fue estadísticamente significativo (<0,0001). Este estudio contribuyó al conocimiento sobre la estructura anatómica de la población estudiada, donde la radiografía panorámica digital es una herramienta importante para el diagnóstico de alteraciones en la región maxilomandibular. Os seios maxilares, estrutura anatômica visualizada radiograficamente por uma ampla área radiolúcida na região posterior de maxila e são considerados como as maiores das cavidades paranasais, podendo ser localizados em radiografias panorâmicas digitais. Nessa área podem ser encontradas extensões, septos intrassinusais e algumas alterações patológicas. Essa pesquisa objetiva investigar e determinar a prevalência dos principais tipos de alterações anatômicas e patológicas em seios maxilares com suas exatas localizações através de radiografias panorâmicas digitais do banco de imagens da Clínica de Radiologia da Universidade Tiradentes (UNIT). A amostra foi composta por 878 exames, totalizando em 1756 seios maxilares. Para avaliar a associação entre as variáveis, utilizou o teste não paramétrico de associação Qui-quadrado, com nível de significância estatística em p<0,05. Para verificar o grau de concordância entre os examinadores foi utilizado o teste estatístico Kappa (p<0,001). Os dados obtidos mostraram que houve uma predominância ao gênero feminino (66,2%). Observou-se maior número de alterações entre indivíduos de 46 a 55 anos (84,78%). Referente às alterações anatômicas, os septos intra sinusais (69,2%) apresentaram-se em maior frequência e, como alterações patológicas, o cisto de retenção de muco (1,17%). Foi observado diferença estatística entre a presença de alterações sinusais em relação à faixa etária (p<0,0001). Houve maior prevalência de alterações sinusais em mulheres, sendo estatisticamente significante (<0,0001). Esse estudo contribuiu para o conhecimento sobre a estrutura anatômica da população estudada, onde a radiografia panorâmica digital, trata-se de uma ferramenta importante para o diagnóstico de alterações na região maxilomandibular.
