Immunological mechanisms of SARS-CoV-2 infection in gestational complications

Resumo: Objective: To analyze and synthesize the main immunological mechanisms related to greater severity or worse outcome in pregnant women infected with SARS-CoV-2. Methods: This is a systematic literature review that was performed by searches on Medline (by PubMed), from October 2021 to January 2022, using the descriptors Pregnancy complications; Immunological Factors; SARS-CoV-2, Covid-19, which aimed to answer the question: “Which immunological mechanisms are related to greater severity or worse outcome in pregnant women infected with SARS-CoV-2?”. Original manuscripts, written in English language and published from 2020 onwards were chosen. Reviews or meta-analyses, editorials and duplicate titles were excluded. The search resulted in 189 articles, of which 11 were selected for matching the eligibility criteria. Results: Changes were found in the immunological functioning of the pregnant organism that may predispose the pregnant woman to a greater susceptibility to SARS-CoV-2 infection and to a more severe course of the disease. These alterations include reduced levels of Natural Killer cells and T helper cells type 1, in addition to an increase in the hyperinflammatory state related to excessive activation of the complement system and an increase in pro-inflammatory cytokines, such as interleukin-6 and α tumor necrosis factor. Besides the increase in angiotensin-converting enzyme 2 in maternal circulation leads to increased viral binding in the host cell and infectivity. Conclusion: Due to the impact that immunological mechanisms have on the evolution of COVID-19 disease on maternal health, it is evident the importance of identifying the elevation of these immunological biomarkers, which can help in the prediction of maternal prognosis and in the definition of appropriate early treatment.

Métodos: Esta es una revisión sistemática de la literatura que realizó búsquedas en Medline (vía PubMed), desde octubre de 2021 hasta enero de 2022, con los descriptores Complicaciones del embarazo; factores inmunológicos; SARS-CoV-2, Covid-19, que buscaba responder a la pregunta: “¿Qué mecanismos inmunológicos se relacionan con mayor gravedad o peor evolución en gestantes infectadas por SARS-CoV-2?”. Se eligieron títulos originales, escritos en inglés y publicados a partir de 2020. Se excluyeron revisiones o metaanálisis, editoriales y títulos duplicados. La búsqueda resultó en 189 artículos, de los cuales 11 fueron seleccionados por cumplir con los criterios de elegibilidad. Resultados: Se encontraron cambios en el funcionamiento inmunológico del organismo gestante que pueden predisponer a la gestante a una mayor susceptibilidad al SARS-CoV-2 y a un curso más severo de la enfermedad. Estas alteraciones incluyen niveles reducidos de células Natural Killer y células T helper tipo 1, además de un aumento del estado hiperinflamatorio relacionado con una activación excesiva del sistema del complemento y un aumento de citocinas proinflamatorias, como la interleucina-6 y la necrosis tumoral. factor -α. Elegeram-se títulos originais, escritos na língua inglesa e publicados a partir de 2020. Excluíram-se revisões ou metanálises, editoriais e títulos duplicados. A busca resultou em 189 artigos, dos quais 11 foram selecionados por preencherem os critérios de elegibilidade. Resultados: Foram encontradas alterações no funcionamento imunológico do organismo grávido que podem predispor a gestante a uma maior suscetibilidade ao SARS-CoV-2 e ao curso mais grave da doença. Incluem-se nessas alterações os níveis reduzidos de células Natural Killer e células T auxiliares do tipo 1, além do aumento do estado hiperinflamatório relacionado à ativação excessiva do sistema complemento e aumento de citocinas pró-inflamatórias, como interleucina-6 e fator de necrose tumoral-α.
