Occlusal plane alteration and its implications for the facial profile of patients submitted to bimaxillar orthognathic surgery – A literature review and report of two clinical cases

Resumo: The aim of the present study was to examine and highlight the effects of orthognathic surgery associated with alteration of the occlusal plane through the correlation of two case reports and a literature review. Through the use of electronic databases, a literature search was carried out, surveying the published studies, that was ilustrated through two case reports. Using surgical approaches, was possible to, basically, promote mandibular advancement and counterclockwise rotation of the occlusal plane in class II patients, even as the mandibular recoil movement with occlusal plane clockwise rotation in the case of a class III patient. All surgical management was carried out in order to create a stable and functional occlusal relationship for patients at a level that could not be reached only by conventional technique. In addition, the study is also applied as implications of this orthosurgical conduct in the tissue changes and facial profile of class II and class III patients, both with vertical excess of the maxilla. It was observed this type of orthosurgical treatment increases postoperative occlusal stability and produces satisfactory aesthetic results.

Mediante el uso de bases de datos electrónicas, se buscó en la literatura encuestando estudios publicados, que fueron ilustrados por dos casos clínicos. Mediante diferentes abordajes quirúrgicos, el objetivo fue básicamente promover el avance mandibular y la rotación del plano mandibular en sentido antihorario en el paciente clase II y el retroceso mandibular con rotación en sentido horario del plano oclusal en el caso del paciente clase III. Todo el manejo quirúrgico se realizó con el fin de proporcionar una relación oclusal estable y funcional para los pacientes a un nivel que no podría lograrse con la técnica convencional sola. Además, el estudio también evaluó las implicaciones de este abordaje ortoquirúrgico para los cambios en los tejidos blandos y el perfil facial de pacientes de clase II y clase III, ambos con exceso vertical del maxilar. Se observó que este tipo de abordaje ortoquirúrgico aumenta la estabilidad oclusal posoperatoria y produce resultados estéticos satisfactorios.O objetivo do trabalho foi examinar e ressaltar os efeitos da cirurgia ortognática associada à alteração de plano oclusal através da correlação de casos clínicos com a revisão de literatura. Através do uso de bases de dados eletrônicas, pesquisou-se na literatura levantando estudos publicados, que foram ilustrados por dois casos clínicos. Lançando mão de diferentes abordagens cirúrgicas, objetivou-se basicamente promover avanço mandibular e rotação do plano mandibular no sentido anti-horário no paciente classe II e recuo mandibular com rotação horária do plano oclusal no caso da paciente classe III. Todo o manejo cirúrgico foi realizado a fim de proporcionar uma relação oclusal estável e funcional para os pacientes em um nível que poderia não ser obtido apenas pela técnica convencional. Além disso, o trabalho também avaliou as implicações dessa conduta orto-cirúrgica nas mudanças do tecido mole e de perfil facial dos pacientes classe II e classe III, ambos com excesso vertical de maxila.
