Evaluation of the impact of diabetes education via social media on glycemic control of patients with type 1 Diabetes mellitus during the COVID-19 pandemic

Resumo: Introduction: Diabetes mellitus (DM) is an important public health problem worldwide. Diabetes structured education must be part of treatment. Objective: Evaluate the use of a social media-based diabetes education and its impact on glucose control. Methods: Randomized clinical trial of type 1 DM outpatients followed at a public health reference clínic of Federal District, Brazil. Participants were randomized to receive diabetes education vídeo via social media (WhatsApp®) or educational information during outpatient consultations, for a period of 12 weeks. The Diabetes Knowledge Scale Questionnaire was applied and results of glycated hemoglobin (HbA1c) and data from glucose managemenst softwares were collected before and after the intervention. Results: 193 participants completed the study, 95 from the intervention group and 98 from the control group. There was an improvement in the questionnaire score in both groups: intervention (p < 0.001) and control (p = 0.021). Regarding glucose control, there was no statistically significant difference when comparing HbA1c and electronic glucose management data between the two groups. Conclusion: It was not possible to establish a relationship of superiority of the strategy used, compared to educational information during outpatient consultations, in improving knowledge about DM. The collected data also suggest that the availability of educational material via WhatsApp® alone does not promote improvement in glucose control.

La educación estructurada aplicada a las personas con DM debe ser parte del tratamiento. Objetivo: Evaluar el uso de una red social como medio de educación en DM y su impacto en el control de la glucosa. Métodos: Ensayo clínico aleatorizado con una muestra compuesta por pacientes con DM tipo 1 seguidos en un ambulatorio público del Distrito Federal, Brasil. Los participantes fueron aleatorizados para recibir videoclases sobre educación diabética a través de la red social WhatsApp® o información educativa durante consultas ambulatorias durante un período de 12 semanas. Se aplicó un cuestionario (Diabetes Knowledge Scale Questionnaire) y se recogieron los resultados de la hemoglobina glucosilada (HbA1c) y los datos de los medidores de glucosa, antes y después de la intervención. Resultados: 193 participantes completaron el estudio, 95 del grupo de intervención y 98 del grupo control. Hubo una mejora en la puntuación del cuestionario en ambos grupos: intervención (p < 0,001) y control (p = 0,021). No hubo diferencias estadísticamente significativas al comparar los datos de gestión electrónica de glucosa y HbA1c entre los dos grupos. Conclusión: No fue posible establecer una relación de superioridad de la estrategia utilizada, en comparación con la información educativa durante las consultas, en la mejora del conocimiento sobre la DM. Métodos: Trata-se de um ensaio clínico randomizado, com amostra constituída de pacientes com DM tipo 1 acompanhados em ambulatório público do Distrito Federal, Brasil. Os participantes foram randomizados para receber videoaulas sobre educação em diabetes via rede social WhatsApp® ou informações educativas durante consultas ambulatoriais, por um período de 12 semanas. Foi aplicado um questionário (Diabetes Knowledge Scale Questionnaire), coletados resultados de hemoglobina glicada (HbA1c) e dados de dispositivos de aferição de glicose, antes e após intervenção. Resultados: Finalizaram o estudo 193 participantes, 95 do grupo intervenção e 98 do grupo controle. Observou-se melhora na pontuação do questionário em ambos os grupos: intervenção (p < 0,001) e controle (p = 0,021). Em relação ao controle da glicose, não houve diferença estatisticamente significativa quando se comparou HbA1c e dados do gerenciamento eletrônico de glicose, entre os dois grupos. Conclusão: Não foi possível estabelecer relação de superioridade da estratégia utilizada, em comparação com informações educativas durante as consultas, na melhora do conhecimento em DM.
