Risk factors and good practices in delirium management: understanding the nursing team

Resumo: Objective: To analyze the nursing team's understanding of risk factors and good practices related to the prevention of delirium in critically ill patients. Methods: Cross-sectional study conducted with the nursing team of Intensive Care Units of a tertiary and university hospital in Porto Alegre/RS. Results: It was found that a large part of the nursing team is aware of the risk factors for delirium in the ICU, such as noise, excessive sedation, increased risk of infection, increased permanence and mortality. Likewise, they understand the benefits of good practices, such as promoting comfort, adequate sedation, presence of a family member, guidance and daily evaluations through delirium scales. Conclusion: It is noticed that the nursing team understands delirium in critically ill patients, the conditions that favor its development, its impact on the length of stay in the ICU and mortality. However, it is observed that improvements are still needed regarding good practices in the management of delirium.

Métodos: estudio transversal realizado con el equipo de enfermería de las unidades de cuidados intensivos de un hospital terciario y universitario en porto alegre/rs. Resultados: se descubrió que gran parte del equipo de enfermería conoce los factores de riesgo de delirio en la uci, como ruido, sedación excesiva, mayor riesgo de infección, mayor permanencia y mortalidad. asimismo, entienden los beneficios de las buenas prácticas, como promover la comodidad, la sedación adecuada, la presencia de un miembro de la familia, la orientación y las evaluaciones diarias a través de escalas de delirio. Conclusión: se observa que el equipo de enfermería comprende el delirio en pacientes críticos, las condiciones que favorecen su desarrollo, su impacto en la duración de la estadía en la uci y la mortalidad. sin embargo, se observa que aún se necesitan mejoras con respecto a las buenas prácticas en el manejo del delirio.Objetivo: Analisar a compreensão da equipe de enfermagem sobre os fatores de risco e as boas práticas relacionadas à prevenção do delirium em pacientes críticos. Métodos: Estudo transversal realizado com a equipe de enfermagem de Unidades de Terapia Intensiva de um hospital terciário e universitário de Porto Alegre/RS. Resultados: Verificou-se que grande parte da equipe de enfermagem tem ciência dos fatores de risco para delirium existentes na UTI, como barulho, sedação excessiva, maior risco de infecção, aumento da permanência e mortalidade. Da mesma forma, entendem os benefícios de boas práticas, como promover conforto, sedação adequada, presença de familiar, orientação e avaliações diárias através de escalas de delirium. Conclusão: Percebe-se que a equipe de enfermagem compreende o delirium no paciente crítico, as condições que propiciam o seu desenvolvimento, o impacto deste no tempo de permanência na UTI e mortalidade.
