Inspiratory muscle strength and lung function in patients with heart failure and obstructive sleep apnea

Resumo: Aim: Assessing inspiratory muscle strength and lung function in patients with chronic heart failure (CHF) and obstructive sleep apnea (OSA). Methods: Cross-sectional study carried out with 18 CHF participants (12 men) who presented reduced left ventricular ejection fraction (LVEF) (< 51% for men and < 53% for women), and who also had OSA (from light to severe), which was diagnosed through portable polysomnography exam. Inspiratory muscle strength was measured by determining the maximum inspiratory pressure (Ipmax) through manovacuometry; it was compared to values predicted by sex and age. Forced vital capacity (FVC), forced expiratory volume in one second (FEV1) and tiffenau index (FVC/FEV1) were measured in computerized spirometer; they were compared to predicted values. Results: Inspiratory muscle strength of CHF and OSA (109 ± 29 cmH2O) patients was preserved in comparison to values predicted by sex and age (100 ± 13cmH2O; p= 0.22). Both, CVF and VEF1 recorded lower values in comparison to the predicted ones (p= 0.001). Conclusions: CHF and OSA patients presented preserved inspiratory muscle strength and reduced lung function in comparison to reference values.

Conclusions: CHF and OSA patients presented preserved inspiratory muscle strength and reduced lung function in comparison to reference values.Objetivo: Evaluar la fuerza de los músculos inspiratorios y la función pulmonar en pacientes con insuficiencia cardíaca crónica (ICC) y apnea obstructiva del sueño (AOS). Métodos: Estudio transversal con 18 participantes con ICC (12 hombres) presentaron fracción de eyección del ventrículo izquierdo (FEVI) reducida (< 51% para los hombres y < 53% para las mujeres), afectado por AOS (leve a grave) diagnosticado mediante polisomnografía portátil. La fuerza muscular inspiratoria fue medida por la determinación de la presión inspiratoria máxima (Pimáx) por manovacuometría y comparado con los valores predichos para el sexo y la edad. La capacidad vital forzada (CVF), el volumen espiratorio forzado en el primer segundo (VEF1)   y el índice de tiffenau (CVF/VEF1) se midieron utilizando un espirómetro computarizado y se compararon con los valores predichos.  Resultados: Se preservó la fuerza muscular inspiratoria de los pacientes con ICC y AOS (109 ± 29 cmH2O) en comparación con los valores predichos para sexo y edad (100 ± 13cmH2O; p= 0,22). Tanto la CVF como la VEF1 fueron disminuidas en relación con los valores predichos (p= 0,001). Conclusiones: Los pacientes con ICC y AOS presentaron fuerza muscular inspiratoria preservada y disminución de la función pulmonar con relación a los valores de referencia.Objetivo: Avaliar a força dos músculos inspiratórios e a função pulmonar em pacientes com insuficiência cardíaca crônica (ICC) e apneia obstrutiva do sono (AOS). Métodos: Estudo transversal com 18 participantes com ICC (12 homens) apresentando fração de ejeção do ventrículo esquerdo (FEVE) reduzida (< 51% para homens e < 53% para mulheres), acometidos por AOS (leve a grave) diagnosticada através do exame de polissonografia portátil. A força muscular inspiratória foi mensurada pela determinação da pressão inspiratória máxima (Pimáx) através de manovacuometria e comparado aos valores preditos para o sexo e idade. A capacidade vital forçada (CVF), volume expiratório forçado no primeiro segundo (VEF1) e índice de tiffenau (CVF/VEF1) foram mensurados através de um espirômetro computadorizado e foram comparados aos valores preditos. Resultados: A força muscular inspiratória dos pacientes com ICC e AOS (109 ± 29 cmH2O) encontrou-se preservada em comparação aos valores preditos para o sexo e idade (100 ± 13cmH2O; p= 0,22).
