Main morphofunctional alterations of the human urinary tract: an integrative literature review

Resumo: Introduction: the morphological alterations of the urinary tract can be subdivided into four main groups: renal anomalies in terms of shape and position; number and volume renal anomalies; collecting system anomalies and urinary bladder anomalies. Objective: to elucidate the main morphofunctional changes in the human urinary tract. Methodology: this is an integrative literature review. The research was carried out through online access to the National Library of Medicine (PubMed MEDLINE), Scientific Electronic Library Online (Scielo), Cochrane Database of Systematic Reviews (CDSR), Google Scholar, Virtual Health Library (BVS) and EBSCO databases. Information Services. Results and Discussion: The first group of alterations includes abnormally rotated kidneys, renal ectopia and renal fusion. These alterations, which are relatively frequent, always culminate in inefficient urine drainage, a factor that can determine a secondary ureteric morphological alteration due to the development of hydronephrosis, for example. The second set of changes are less common and more independent than those relating to kidney shape and position. In this sense, the following can be mentioned: supernumerary kidneys, which are usually unilateral and not hereditary; renal hypoplasia; renal agenesis; and cystic disease, which occurs due to a failure in the genesis of the junction that would allow urine to pass from the glomerulus to the renal pelvis. It is worth remembering that, with age, there is a physiological loss of renal tissue, which can make an individual, even if healthy, fit into the group of people with morphological changes in volume. Conclusion: renal anomalies in terms of shape and position, renal anomalies in number and volume, anomalies of the collecting system and anomalies of the urinary bladder must be studied and well understood by professionals, so that it is possible to recognize such changes and understand how to approach them.

Objetivo: dilucidar los principales cambios morfofuncionales en el tracto urinario humano. Metodología: se trata de una revisión integrativa de la literatura. La investigación se llevó a cabo a través del acceso en línea a la Biblioteca Nacional de Medicina (PubMed MEDLINE), Scientific Electronic Library Online (Scielo), Cochrane Database of Systematic Reviews (CDSR), Google Scholar, Virtual Health Library (BVS) y las bases de datos EBSCO. Servicios. Resultados y Discusión: El primer grupo de alteraciones incluye riñones anormalmente rotados, ectopia renal y fusión renal. Estas alteraciones, que son relativamente frecuentes, culminan siempre en un drenaje urinario ineficiente, factor que puede determinar una alteración morfológica ureteral secundaria por el desarrollo de hidronefrosis, por ejemplo. El segundo conjunto de cambios es menos común y más independiente que los relacionados con la forma y la posición de los riñones. En este sentido, se pueden mencionar los siguientes: riñones supernumerarios, que suelen ser unilaterales y no hereditarios; hipoplasia renal; agenesia renal; y la enfermedad quística, que se produce por un fallo en la génesis de la unión que permitiría el paso de la orina desde el glomérulo hasta la pelvis renal. Vale recordar que, con la edad, se produce una pérdida fisiológica del tejido renal, que puede hacer que un individuo, aunque sea sano, encaje en el grupo de personas con cambios morfológicos de volumen. Conclusión: las anomalías renales en forma y posición, las anomalías renales en número y volumen, las anomalías del sistema colector y las anomalías de la vejiga urinaria deben ser estudiadas y bien comprendidas por los profesionales, de modo que sea posible reconocer tales cambios y comprender cómo para acercarme a ellos.Introdução: as alterações morfológicas do trato urinário podem ser subdivididas em quatro principais grupos: anomalias renais quanto à forma e à posição; anomalias renais de número e de volume; anomalias do sistema coletor e anomalias da bexiga urinária. Objetivo: elucidar acerca das principais alterações morfofuncionais do trato urinário humano. Metodologia: trata-se de uma revisão integrativa de literatura. A pesquisa foi realizada através do acesso online nas bases de dados National Library of Medicine (PubMed MEDLINE), Scientific Electronic Library Online (Scielo), Cochrane Database of Systematic Reviews (CDSR), Google Scholar, Biblioteca Virtual em Saúde (BVS) e EBSCO Information Services. Resultados e discussão: o primeiro grupo de alterações engloba rins rotacionados anormalmente, ectopia renal e fusão renal. Essas alterações, as quais são, relativamente, frequentes, sempre culminam em uma drenagem de urina ineficiente, fator que pode determinar uma alteração morfológica uretérica secundária em virtude do desenvolvimento de uma hidronefrose, por exemplo. O segundo conjunto de alterações são menos comuns e mais independentes dos que as relativas à forma e à posição renais. Nesse sentido, podem-se citar: rins supranumerários, que geralmente são unilaterais e não hereditários; hipoplasia renal; agenesia renal; e doença cística, que ocorre devido a uma falha na gênese da junção que permitiria que a urina passasse do glomérulo à pelve renal. Vale lembrar que, com a idade, há uma perda fisiológica de tecido renal, o que pode fazer com que um indivíduo, ainda que saudável, venha a se encaixar no grupo de pessoas com alterações morfológicas de volume.
