Prevalence and factors associated with food insecurity among adults with HIV/AIDS treated at a referral hospital in Mozambique

Resumo: Food insecurity is a public health problem due to its effects on the health of people infected with HIV, despite the undeniable benefits of antiretroviral treatment. The study aimed to analyze the prevalence and factors associated with food insecurity in people living with HIV/AIDS. This is a cross-sectional study, conducted with 206 patients seen, in May-July 2022. Socioeconomic, demographic, lifestyle and food security data were collected. Food insecurity was assessed using the Brazilian Scale of Food Insecurity, and its associations through Pearson's chi-square test/Fisher's exact test and prevalence ratio. The prevalence of food insecurity (severe) was higher in males (78.9%), aged 18-30 years (88.9%), with no level of education (82.4%), unemployed (72.3%), married (73.1%), in households of 4 to 6 members (77, 3%) and living in inadequate housing conditions (70.2%), with stage II disease (78.4%), non-smokers (71.9%), regular consumers of alcoholic beverages (73.7%) and practicing physical activity (76.4%). Statistical associations were found between food insecurity and age group (p = 0.001), education (p = 0.001), employment status (p < 0.011), number of people per household (p < 0.028), being a smoker (p < 0.013) and disease stage (p < 0.017). The results suggest that investments in policies aimed at improving agricultural production, income, education, access to employment and reduction of poverty levels are necessary to increase the availability, improved access and use of food.

El estudio pretendía analizar la prevalencia y los factores asociados a la inseguridad alimentaria en las personas que viven con el VIH/SIDA. Se trata de un estudio transversal, realizado con 206 pacientes atendidos, en mayo-julio de 2022. Se recogieron datos socioeconómicos, demográficos, de estilo de vida y de seguridad alimentaria. La inseguridad alimentaria se evaluó a través de la Escala Brasileña de Inseguridad Alimentaria, y sus asociaciones a través de la prueba de chi-cuadrado de Pearson/prueba exacta de Fisher y la razón de prevalencia. La prevalencia de la inseguridad alimentaria (severa) fue mayor en los hombres (78,9%), con edades comprendidas entre los 18 y los 30 años (88,9%), sin nivel de estudios (82,4%), desempleados (72,3%), casados (73,1%), en hogares de 4 a 6 miembros (77, 3%) y vivir en condiciones de vivienda inadecuadas (70,2%), con enfermedad en estadio II (78,4%), no fumador (71,9%), consumidores habituales de bebidas alcohólicas (73,7%) y practicantes de actividad física (76,4%). Se encontraron asociaciones estadísticas entre la inseguridad alimentaria y el grupo de edad (p = 0,001), la educación (p = 0,001), la situación laboral (p < 0,011), el número de personas por hogar (p < 0,028), ser fumador (p < 0,013) y el estadio de la enfermedad (p < 0,017). Los resultados sugieren que es necesario invertir en políticas destinadas a mejorar la producción agrícola, los ingresos, la educación, el acceso al empleo y la reducción de los niveles de pobreza para aumentar la disponibilidad, la mejora del acceso y la utilización de los alimentos.Introdução: A insegurança alimentar é um problema de saúde púbica, devido aos seus efeitos na saúde de pessoas infectadas pelo HIV, apesar dos benefícios indiscutíveis do tratamento antirretroviral. Objetivo: analisar a prevalência e fatores associados à insegurança alimentar em pessoas que vivem com HIV/AIDS. Metodologia: Trata-se de um estudo transversal, realizado com 206 pacientes atendidos, em maio-julho de 2022. Foram coletados dados socioeconômicos, demográficos, de estilo de vida e de segurança alimentar. Avaliou-se a insegurança alimentar por meio da Escala Brasileira de Insegurança Alimentar, e as suas associações através do teste qui-quadrado de Pearson/teste exato de Fisher e razão de prevalência. Resultados: A prevalência de insegurança alimentar (grave) foi mais elevada em homens (78,9%), com idade de 18-30 anos (88,9%), sem nenhum nível de escolaridade (82,4%), desempregados (72,3%), casados (73,1%), em agregados de 4 a 6 membros (77,3%) e morando em condições de habitação inadequada (70,2%), com estádio II da doença (78,4%), não fumantes (71,9%), consumidores regulares de bebidas alcoólicas (73,7%) e que praticavam atividade física (76,4%). Encontrou-se associações estatísticas entre insegurança alimentar e faixa etária (p = 0,001), escolaridade (p = 0,001), situação de emprego (p < 0,011), número de pessoas por domicílio (p < 0,028), ser fumante (p < 0,013) e estádio da doença (p < 0,017).
