Periodontitis and pulmonary repercussions: Is the casual relationship between them well clarified in the literature?

Resumo: Introduction: The oral cavity is a potential reservoir of respiratory pathogens. The mechanisms of infection that can be cited are the aspiration of bacteria that normally reside in the oropharynx, the colonization of dental plaque by respiratory pathogens followed by aspiration, and the ease of colonization of periodontal pathogens in the upper airway, thus providing a link between periodontitis and respiratory/pulmonary conditions also of an inflammatory nature. However, the causal relationship between periodontitis and the development of pulmonary disorders is not well understood. This research aims to verify the advances in the discussions of the relationship between periodontitis and pulmonary diseases. Metodology: This is an sistematic literature review study. The guiding question of the research was elaborated using the strategy of the acronym PICO. Searches for eligible studies were carried out from January 31, 2022 to February 16, 2022, in the databases: PubMed and BVS Library, using elements from the PICO acronym to obtain specific and controlled descriptors, available in Descriptors in Health Sciences/Medical Subject Headings (DeCS/MeSH): periodontitis, inflammation, lung diseases, which were combined during the search by the boolean AND operator. Case reports, clinical study, clinical trial, observational study, randomized controlled trial, available in English, Chinese and Portuguese, were selected, attributing a time cut of 12 years (2010-2022). Inclusion criteria were: original and review articles whose main subject was related to periodontitis and its relationship with pulmonary disorders. The following were excluded: duplicate works, incomplete works, studies that omitted the sample size, study protocols, articles outside the time frame of the research, works without relevant results or out of context, articles whose research addressed studies in animals and articles with conflict of interest. interests among the authors. 69 articles were identified in the databases defined for the search. After the process of selection and analysis of the works found, 21 articles were selected for review. Results and discussion: The results are organized according to the presentation of the sample variables (sample size, characteristics of the participants regarding: age, sex, presence of periodontitis, presence of COPD, presence of periodontitis concomitant with COPD), research design, type of intervention (description of procedures in relation to treatment, assessment of periodontal status and respiratory functions), type of control for the treatment group, and description of the main outcomes (changes in quality of life, oral hygiene status and functioning of respiratory functions evaluated in the participants, before and after the intervention). Conclusion: The relationship between periodontitis and lung conditions is well elucidated in the literature, especially in patients who have both conditions (periodontitis and lung disorders). However, it can also be observed that there is no clear and substantial evidence that explains the causal mechanisms between them. This review alludes and points to better clarifications in this field.

Esta investigación tiene como objetivo verificar los avances en las discusiones de la relación entre la periodontitis y las enfermedades pulmonares. Metodología: Estudio de revisión sistemática de la literatura. La pregunta guía de la investigación fue elaborada utilizando la estrategia de las siglas PICO. Se realizaron búsquedas de estudios elegibles desde el 31 de enero de 2022 hasta el 16 de febrero de 2022, en las bases de datos: PubMed y BVS Library, utilizando elementos de la sigla PICO para obtener descriptores específicos y controlados, disponibles en Descriptors in Health Sciences/Medical Subject Headings (DeCS/MeSH): periodontitis, inflamación, enfermedades pulmonares, que se combinaron durante la búsqueda mediante el operador booleano AND. Se seleccionaron informes de casos, estudios clínicos, ensayos clínicos, estudios observacionales, ensayos controlados aleatorizados, disponibles en inglés, chino y portugués, atribuyéndoles un corte de tiempo de 12 años (2010-2022). Los criterios de inclusión fueron: artículos originales y de revisión cuyo tema principal estuviera relacionado con la periodontitis y su relación con los trastornos pulmonares. Se excluyeron: trabajos duplicados, trabajos incompletos, estudios que omitieron el tamaño de la muestra, protocolos de estudio, artículos fuera del marco temporal de la investigación, trabajos sin resultados relevantes o fuera de contexto, artículos cuya investigación abordó estudios en animales y artículos con conflicto de interés intereses entre los autores. Se identificaron 69 artículos en las bases de datos definidas para la búsqueda. Luego del proceso de selección y análisis de los trabajos encontrados, se seleccionaron 21 artículos para su revisión. Resultados y discusión: Los resultados están organizados de acuerdo a la presentación de las variables de la muestra (tamaño de la muestra, características de los participantes en cuanto a: edad, sexo, presencia de periodontitis, presencia de EPOC, presencia de periodontitis concomitante a EPOC), diseño de la investigación , tipo de intervención (descripción de procedimientos en relación con el tratamiento, evaluación del estado periodontal y funciones respiratorias), tipo de control para el grupo de tratamiento, y descripción de los principales resultados (cambios en la calidad de vida, estado de higiene bucal y funcionamiento de funciones respiratorias evaluadas en los participantes, antes y después de la intervención). Conclusión: La relación entre la periodontitis y los trastornos pulmonares está bien aclarada en la literatura, especialmente en pacientes que presentan ambas condiciones (periodontitis y trastornos pulmonares). Sin embargo, también se puede observar que no existe evidencia clara y sustancial que explique los mecanismos causales entre ellos. Esta revisión alude y apunta a mejores aclaraciones en este campo.Introdução: A cavidade oral é um potencial reservatório de patógenos respiratórios. Os mecanismos de infecção que podem ser citados são a aspiração de bactérias que normalmente residem na orofaringe, a colonização da placa dentária por patógenos respiratórios seguida da aspiração e a facilidade da colonização dos patógenos periodontais na via aérea superior, proporcionando, assim, uma ligação entre periodontite e afecções respiratórias/pulmonares também de caráter inflamatório. Contudo, a relação de causalidade entre a periodontite e o desenvolvimento de afecções pulmonares não é bem esclarecida. Essa pesquisa tem como objetivo verificar os avanços das discussões da relação entre periodontite e afecções pulmonares. Metodologia: Estudo de revisão sistemática da literatura. A questão norteadora da pesquisa foi elaborada utilizando a estratégia do acrônimo PICO. As buscas de estudos elegíveis foram realizadas no período de 31 de janeiro de 2022 a 16 de fevereiro de 2022, nas bases de dados: PubMed e Biblioteca BVS, utilizando os elementos oriundos do acrônimo PICO para obtenção de descritores específicos e controlados, disponíveis no Descritores em Ciências da Saúde/Medical Subject Headings (DeCS/MeSH): periodontite, inflamação, doenças pulmonares, que foram combinados durante a busca pelo operador booleano AND. Foram selecionados relatos de casos, estudos clínicos, ensaios clínicos, estudos observacionais, ensaios randomizados controlados, disponíveis em inglês, chinês e português, atribuindo-se o recorte de tempo de 12 anos (2010- 2022). Foram considerados critérios de inclusão: artigos originais e de revisão cujo assunto principal estivesse relacionado à periodontite e sua relação com afecções pulmonares. Foram excluídos: trabalhos duplicados, trabalhos incompletos, trabalhos que omitiram o tamanho da amostra, protocolos de estudo, artigos fora do recorte temporal da pesquisa, trabalhos sem resultados relevantes ou fora de contexto, artigos cuja pesquisa abordou estudos em animais e artigos com conflito de interesses entre os autores. Foram identificados 69 artigos nas bases de dados definidas para a busca. Após o processo de seleção e análise dos trabalhos encontrados, foram selecionados para a revisão 21 artigos. Resultados e discussão: Os resultados estão organizados de acordo com a apresentação das variáveis da amostra (tamanho amostral, características dos participantes quanto: a idade, o sexo, presença de periodontite, presença de DPOC, presença de periodontite concomitante à DPOC), desenho da pesquisa, tipo de intervenção (com a descrição dos procedimentos em relação ao tratamento, avaliação do estado periodontal e às funções respiratórias), tipo de controle ao grupo de tratamento, e descrição dos principais desfechos (alterações da qualidade de vida, dos estados de higiene bucal e do funcionamento das funções respiratórias avaliadas nos participantes, na pré e pós intervenção). Conclusão: A relação entre a periodontite e as afecções pulmonares é bem elucidada na literatura, sobretudo em pacientes que apresentam ambas as condições (periodontite e afecções pulmonares). No entanto, pode-se observar também que inexiste evidências claras e substanciais que expliquem os mecanismos de causalidade entre elas.
