Manejo da osteomielite em ferida de adultos: Aspectos terapêuticos

Resumo: Objectives: to identify in the scientific literature therapeutic methods used for osteomyelitis in chronic wounds in adults and to develop a clinical guide for a federal University Hospital in Rio de Janeiro. Methodology: integrative literature review, whose inclusion criteria were complete articles on therapeutic aspects of osteomyelitis in adult wounds since 2018, in Portuguese, Spanish or English. The systematic electronic search occurred using descriptors in health sciences (DeCS) and Medical Subject Heading (MeSH). Results: 508 articles identified (500 electronic and 08 physical), after reading the abstract by two researchers and excluding duplicates, 76 articles remained selected and included for analysis. The majority of studies were American (42.10%) and published by doctors (92.1%). 98% pointed to microbiological culture and blood tests as guides for therapeutic parameters, in addition to imaging tests as complementary (20%). Surgical debridement was the most used procedure (82.3%) to remove bone infection. There was no therapeutic consensus, but recommendations for good practices, which are included in the clinical guide for treating wounds with osteomyelitis prepared based on this review. Conclusion: the lack of consensus and the scarcity of literature, especially nationally, may be compromising safety and quality of care. Therefore, nurses, especially specialists in dermatological nursing/stomatherapy, must exercise leadership in early clinical assessment with a view to referrals for medical diagnostic confirmation . And with this, accelerate the initiation of the most appropriate treatment guided by tests, which could prevent complications and the chronicity of a public health problem.

Metodologia: revisão integrativa da literatura, cujos critérios de inclusão foram artigos completos sobre aspectos terapêuticos da osteomielite em feridas de adultos desde 2018, em português, espanhol ou inglês. A busca eletrônica sistemática ocorreu mediante descritores em ciências da saúde (DeCS) e Medical Subject Heading (MeSH). Resultados: 508 artigos identificados (500 eletrônicos e 08 físicos), após leitura de resumo por dois pesquisadores e excluídas duplicatas, restaram 76 artigos selecionados e incluídos para análise. A maioria dos estudos eram americanos (42,10%) e publicados por médicos (92,1%). 98% apontou a cultura microbiológica e exames de sangue enquanto guias para parâmetros terapêuticos, além de exames de imagens enquanto complementares (20%). O debridamento cirúrgico foi o procedimento mais utilizado (82,3%) para remoção da infecção óssea. Não houve consenso terapêutico, mas recomendações de boas práticas, que constam no guia clínico de tratamento de ferida com osteomielite elaborado com base nesta revisão. Conclusão: a falta de consenso e a escassez literária, especialmente nacional podem estar comprometendo a segurança e qualidade assistencial, diante disso, enfermeiros, especialmente especialistas em enfermagem dermatológica/estomaterapia, devem exercer liderança na avaliação clínica precoce com vistas aos encaminhamentos para confirmação diagnóstica médica. La búsqueda electrónica sistemática se realizó utilizando descriptores en ciencias de la salud (DeCS) y Medical Subject Heading (MeSH). Resultados: Se identificaron 508 artículos (500 electrónicos y 08 físicos), luego de la lectura del resumen por dos investigadores y la exclusión de duplicados, quedaron seleccionados e incluidos para el análisis 76 artículos. La mayoría de los estudios eran americanos (42,10%) y publicados por médicos (92,1%). El 98% señaló el cultivo microbiológico y los análisis de sangre como guías de parámetros terapéuticos, además de las pruebas de imagen como complementarias (20%). El desbridamiento quirúrgico fue el procedimiento más utilizado (82,3%) para eliminar la infección ósea. No hubo consenso terapéutico, pero sí recomendaciones de buenas prácticas, que se incluyen en la guía clínica para el tratamiento de heridas con osteomielitis elaborada a partir de esta revisión. Conclusión: la falta de consenso y la escasez de literatura, especialmente a nivel nacional, pueden estar comprometiendo la seguridad y la calidad de la atención, por lo que los enfermeros, especialmente los especialistas en enfermería dermatológica/estomaterapia, deben ejercer liderazgo en la evaluación clínica temprana con miras a las derivaciones para atención médica. confirmación diagnóstica.
