Unlicensed drugs: a discussion in pediatrics

Resumo: The scarcity of specific industrialized products for the pediatric public can impact the consumption of unlicensed drugs. Unlicensed drugs are those originating from licensed products, through modifications, in addition to imported ones, contraindicated ones and those that do not have information about the use in a certain group. This study aimed to investigate the prevalence of the use of unlicensed drugs, the drugs frequently prescribed in this context and the pharmaceutical forms of greatest production. In this sense, the methodology of integrative review of the scientific literature was used. The prevalence rates for the use of unlicensed medications ranged from 1.6 to 33.35%, demonstrating the diversity of the scenario according to the country studied. The drugs constantly prescribed were diuretics (furosemide, spironolactone and hydrochlorothiazide), folic acid, caffeine, phenobarbital and morphine. The pharmaceutical forms most frequently produced were liquid, with emphasis on solutions, suspensions and syrups. Although the pharmaceutical market has improved its productive capacity over time, the use of unlicensed drugs is still part of the pediatric care scenario in several countries.

Los medicamentos sin licencia son aquellos provenientes de productos licenciados, mediante modificaciones, además de los importados, contraindicados y los que no tienen información sobre el uso en un determinado grupo. Este estudio tuvo como objetivo investigar la prevalencia del uso de medicamentos sin licencia, los medicamentos frecuentemente prescritos en este contexto y las formas farmacéuticas de mayor producción. En este sentido, se utilizó la metodología de revisión integradora de la literatura científica. Las tasas de prevalencia para el uso de medicamentos sin licencia oscilaron entre 1,6 y 33,35%, lo que demuestra la diversidad del escenario según el país estudiado. Los fármacos prescritos constantemente fueron diuréticos (furosemida, espironolactona e hidroclorotiazida), ácido fólico, cafeína, fenobarbital y morfina. Las formas farmacéuticas producidas con mayor frecuencia fueron las líquidas, con énfasis en soluciones, suspensiones y jarabes. Si bien el mercado farmacéutico ha mejorado su capacidad productiva a lo largo del tiempo, el uso de medicamentos sin licencia sigue siendo parte del escenario de la atención pediátrica en varios países.A escassez de produtos industrializados específicos para o público pediátrico pode repercutir o consumo de medicamentos não licenciados. Os medicamentos não licenciados são aqueles originados a partir de produtos licenciados, por meio de modificações, além dos importados, os contraindicados e os que não possuem informações sobre o uso em determinado grupo. Esse estudo objetivou investigar a prevalência de uso de medicamentos não licenciados, os fármacos frequentemente prescritos nesse contexto e as formas farmacêuticas de maior produção. Nesse sentido, foi utilizada a metodologia de revisão integrativa da literatura científica. As taxas de prevalência de uso de medicamentos não licenciados variaram de 1,6 a 33,35%, demonstrando a diversidade do cenário conforme o país estudado. Os fármacos constantemente prescritos foram os diuréticos (furosemida, espironolactona e hidroclorotiazida), o ácido fólico, a cafeína, o fenobarbital e a morfina. As formas farmacêuticas produzidas com maior frequência foram as líquidas, com destaque para as soluções, as suspensões e os xaropes.
