Reflections on the association between smoking and COVID - 19: literature review

Resumo: The pathogenesis of COVID-19 is largely unknown, and data are limited on the clinical characteristics of patients and their prognostic factors, assuming that smoking is possibly associated with the adverse prognosis of the disease. This paper aims to describe, based on the scientific literature, the association between tobacco consumption and the progression of COVID-19. It is an integrative literature review, and searches were carried out in the PUBMED and BIREME database, using the descriptors “tobacco” and “COVID-19”, complete and free articles published in the year 2020. The final sample of this The review consisted of 8 (eight) scientific articles. The main changes related to the progression of COVID-19 in smokers in the studies analyzed were: greater expression of the ACE2 receptor gene in smokers, contributing to the progression of symptoms, increased inflammatory response in subchronic smokers of electronic cigarettes, enrichment the androgenic pathway and the specific positive regulation of the regulatory genes of the central pathway and the realization of campaigns to combat smoking. As long as the related pathophysiological processes are not fully elucidated, it is important to reinforce campaigns to maintain healthy lifestyle habits, among which the non-use of tobacco, even in alternative forms, such as electronic cigarettes, as use is associated with greater predisposition to more severe clinical manifestations of the disease.

Este artículo tiene como objetivo describir, con base en la literatura científica, la asociación entre el consumo de tabaco y la progresión de COVID-19. Se trata de una revisión integradora de la literatura, y se realizaron búsquedas en la base de datos PUBMED y BIREME, utilizando los descriptores “tabaco” y “COVID-19”, de artículos completos y gratuitos publicados en el año 2020. La muestra final de esta revisión consistió en 8 (ocho) artículos científicos. Los principales cambios relacionados con la progresión de COVID-19 en fumadores en los estudios analizados fueron: aumento de la expresión del gen del receptor ACE2 en fumadores, contribuyendo a la progresión de los síntomas, aumento de la respuesta inflamatoria en fumadores subcrónicos de cigarrillos electrónicos, enriquecimiento la vía androgénica y la regulación positiva específica de los genes reguladores de la vía central y la realización de campañas de lucha contra el tabaquismo. Mientras no se esclarezcan por completo los procesos fisiopatológicos relacionados, es importante reforzar las campañas para mantener hábitos de vida saludables, entre los que destaca el no consumo de tabaco, incluso en formas alternativas, como los cigarrillos electrónicos, ya que el consumo se asocia a mayor predisposición a manifestaciones clínicas más graves de la enfermedad.A patogênese do COVID-19 é amplamente desconhecida, e os dados são limitados sobre as características clínicas dos pacientes e seus fatores prognósticos, presumindo -se que o tabagismo esteja possivelmente associado ao prognóstico adverso da doença. Este trabalho tem como objetivo descrever baseando-se na literatura científica a associação entre o consumo de tabaco e a progressão da COVID-19. Trata-se de uma revisão de literatura do tipo integrativa, e realizaram-se buscas na base de dados PUBMED e BIREME, utilizando os descritores “tobacco” and “COVID-19”, de artigos completos e gratuitos publicados no ano de 2020. A amostra final desta revisão foi constituída por 8 (oito) artigos científicos. As principais alterações relacionadas com a progressão da COVID-19 em indivíduos tabagistas nos estudos analisados foram: maior expressão do gene do receptor ACE2 em fumantes, contribuindo com a progressão dos sintomas, o aumento da resposta inflamatória em fumantes subcrônicos de cigarros eletrônicos, o enriquecimento da via androgênica e à regulação positiva específica dos genes reguladores da via central e a realização de campanhas de combate ao tabagismo.
