Prevalence of Temporomandibular Dysfunction in Patients with Dentures and Association with Psychological Factors

Resumo: The term TMD is used to define conditions involving changes in the structure and / or function of the masticatory system involving temporomandibular joint (TMJ) and related musculoskeletal structures. Despite the development of Preventive Dentistry during the last decades and the advent of integrated bone implants, it is still possible to find individuals who require rehabilitation with a conventional full denture. Regardless of the cause of edentulism, TMD can affect total edentulous patients, with or without dentures. Inexplicably, the edentulous individuals have not been considered for a long time, and still, in the present context, the scientific publications with significant relevance, referring to the prevalence of TMD in total edentulous individuals, with or without total dentures, are still scarce and controversial, providing no basis for a consistent and reliable conclusion. Type of study: cross-sectional study. Methods: Fifty patients with total dentures were evaluated. The individuals were grouped according to the clinical and psychological diagnosis through RDC / TMD. It was carried out retention and stability test and vertical dimension. Results: The presence of TMD was found in only 14% of the sample. 86% of patients reported stability and poor retention, normal DVO with normal in 70% patients and 62% of patients were diagnosed with moderate depression. Conclusion: In the present study, the TMD obtained a low percentage in the sample. There was a significant association between the presence of TMD and the degree of depression.

A pesar del desarrollo de la odontología preventiva durante las últimas décadas y el advenimiento de los implantes óseos integrados, todavía es posible encontrar personas que necesiten rehabilitación con dentaduras completas convencionales. Independientemente de la causa del edentulismo, los TMD pueden afectar a pacientes desdentados totales, tengan o no prótesis. Se realizaron pruebas de retención y estabilidad de las prótesis y medidas de la dimensión vertical de la oclusión de los usuarios de prótesis totales. Resultados: La presencia de TMD se encontró en solo el 14% de la muestra mientras que el 86% de los pacientes refirió baja estabilidad y retención de las prótesis, OVD con patrón normal en el 70% de los pacientes y 62% de los pacientes fueron diagnosticados de depresión moderada. Conclusión: No hubo asociación entre TMD y uso de prótesis completas, ya que solo el 14% de la muestra tenía TMD, pero el 62% de los pacientes tuvo asociación entre la presencia de TMD y depresión.O termo DTM é utilizado para definir condições que envolvem alterações da estrutura e/ou função do sistema mastigatório envolvendo a Articulação Temporomandibular (ATM) e estruturas musculoesqueléticas relacionadas. Independentemente da causa do edentulismo, as DTM podem acometer pacientes desdentados totais, portadores ou não de próteses. Inexplicavelmente, por um longo período os indivíduos desdentados não foram considerados, e ainda, no contexto atual, as publicações científicas com significativa relevância, referentes à prevalência de DTM em indivíduos desdentados totais, portadores ou não de próteses totais, ainda são escassas e controversas, não provendo subsídios para uma conclusão consistente. Tipo de estudo: estudo transversal. Objetivo: A partir de um estudo transversal avaliar e investigar a prevalência de DTM em indivíduos desdentados totais. Material e métodos: Foram avaliados 50 pacientes portadores de próteses totais, os quais foram agrupados de acordo com o diagnóstico clínico e psicológico através do RDC/TMD. Foi realizado teste de retenção e estabilidade das próteses e medidas de dimensão vertical de oclusão dos usuários de Prótese totais. Resultados: A presença de DTM foi encontrada em apenas 14% da amostra enquanto 86% dos pacientes relataram baixa estabilidade e retenção das próteses, DVO com padrão de normalidade em 70% dos pacientes e 62% dos pacientes foram diagnosticados com depressão moderada.
