Scientific evidence on access to health services by LGBTQI+ population: Scope review

Resumo: | The relationship between users and health services is essential to strengthening the quality of care. The LGBTI+ population, which is composed of populations that are defined by the diversity of gender identity and expression, sexual orientation, and biological sex, suffers prejudice and discrimination in accessing and using these services. This study aimed to identify the scientific evidence in the LGBTQI+ population, to give visibility to the adversities related to the access and use of health services. Scope review carried out at the bases: CINAHL, EBSCOhost, EMBASE; Latindex; LILACS, MEDLINE, Science Direct, SciELO, SCOPUS, Web of Science, with an open period. Original articles with full text available online in English, Spanish or Portuguese were included, with data from Brazil as the object of study. The search resulted in 1,332 potentially eligible articles and, additionally, 4 more articles from references of articles selected in the research, totaling 1336 articles. Of these, 22 met all inclusion criteria. Evidence has shown that the LGBTQI+ population suffers discrimination in healthcare environments and, as a rule, barriers to access. Stigma, discrimination, and cultural and social norms that give preference to heterosexuality, cisgenderism, and endosexism are some of the causes that contribute to these health inequalities. The results of this study show gaps in cultural competencies concerning health, lack of awareness and knowledge of the unique health needs of the LGBTQI+ population, along with non-inclusive attitudes, were identified as possible contributing factors among health professionals and managers. |

La población LGBTI+, que está compuesta por poblaciones que se definen por la diversidad de identidad y expresión de género, orientación sexual y sexo biológico, sufre prejuicios y discriminación en el acceso y uso de estos servicios. Este estudio tuvo como objetivo identificar la evidencia científica en la población LGBTQI+. Revisión de alcance realizada en las bases: CINAHL, EBSCOhost, EMBASE; Latindex; LILACS, MEDLINE, Science Direct, SciELO, SCOPUS, Web of Science, con período abierto. Se incluyeron artículos originales con texto completo en inglés, español o portugués, con datos de Brasil como objeto de estudio. La búsqueda resultó en 1332 artículos potencialmente elegibles y, adicionalmente, 4 artículos más de referencias de artículos seleccionados en la investigación, totalizando 1336 artículos. De estos, 22 cumplieron con todos los criterios de inclusión. La evidencia ha demostrado que la población LGBTQI+ sufre discriminación en los entornos sanitarios y, por regla general, barreras de acceso. El estigma, la discriminación, las normas culturales y sociales que dan preferencia a la heterosexualidad, el cisgenerismo y el endosexismo son algunas de las causas que contribuyen a estas desigualdades en salud. Los resultados de este estudio muestran brechas en las competencias culturales en relación con la salud, la falta de conciencia y conocimiento de las necesidades de salud únicas de la población LGBTQI+, junto con actitudes no inclusivas, se identificaron como posibles factores contribuyentes entre los profesionales y administradores de la salud.A relação entre usuários e serviços de saúde é essencial para fortalecer a qualidade da assistência. A busca resultou em 1.332 artigos potencialmente elegíveis e, adicionalmente, mais 4 artigos advindos de referências de artigos selecionados na pesquisa, totalizando 1336 artigos. Destes, 22 atenderam a todos os critérios de inclusão. Evidências demonstraram que a população LGBTQI+ sofre discriminação em ambientes de saúde e, via-de-regra, barreiras de acesso. O estigma, a discriminação, as normas culturais e sociais que dão preferência à heterossexualidade, cisgenerismo e endossexismo são algumas das causas que contribuem para essas desigualdades em saúde.
