RESTORATION OF THE LOST VERTICAL DIMENSION DUE TO SEVERE BRUXISM: A CASE REPORT

Resumo: Introduction: One of the dentist’s greatest challenges when planning an oral rehabilitation is to restore the altered vertical dimension, sometimes due to the loss of posterior teeth or caused by their excessive wearing as a consequence of parafunctional habits. Proposition: to present a case report in which the patient’s vertical dimension was re-established after being lost to the habit of bruxism. Case report: A 48-year-old male leucodermic patient had a severe teeth wearing, leading to the loss of the occlusion vertical dimension. Due to such huge loss of tooth structure, the patient had several affected teeth; therefore, endodontic treatments were performed with the subsequent nucleus implantation. To recover the vertical dimension, metal-ceramic crowns were placed in all upper jaw teeth; in the lower arch, only the anterior teeth received metal-ceramic crowns and, due to the absence of posterior teeth, the patient was rehabilitated with a removable partial denture. At the end of treatment, the patient received a (myo-relaxing) interocclusal plate to protect the crowns, providing a more stable joint positioning. Conclusions: It is extremely important that the dentist is able to solve the patients’ problems, attending to their needs, rehabilitating them the best way possible, so as to meet their expectations of a better quality of life.

Conclusions: It is extremely important that the dentist is able to solve the patients’ problems, attending to their needs, rehabilitating them the best way possible, so as to meet their expectations of a better quality of life. Introdução: Um dos maiores desafios para o cirurgião-dentista no momento do planejamento de uma reabilitação bucal é como restabelecer a dimensão vertical alterada, às vezes por perda dos dentes posteriores ou causada por seu desgaste excessivo em consequência de hábitos parafuncionais. Proposição: Relato de caso clínico e a maneira pela qual foi restabelecida a dimensão vertical do paciente, perdida pelo hábito bruxista. Caso clínico: O paciente do sexo masculino, 48 anos, leucoderma, possuía um desgaste severo nos dentes, o que acarretou uma perda da dimensão vertical de oclusão. Em virtude dessa grande perda da estrutura dental, o paciente tinha vários dentes já comprometidos e, por isso, foram realizados tratamentos endodônticos com posterior instalação de núcleos. Para a recuperação da dimensão vertical foram colocadas coroas metalocerâmicas em todos os dentes da arcada superior; já na arcada inferior, somente os dentes anteriores receberam coroas metalocerâmicas e, em virtude da ausência dos dentes posteriores, o paciente foi reabilitado com uma prótese parcial removível. Ao fim do tratamento, o paciente recebeu uma placa interoclusal (mio-relaxante) para proteção das coroas, propiciando posicionamento articular mais estável.
