Manobra de desobstrução de vias aéreas: conhecimento de professores de centros municipais de educação infantil

Resumo: The leading cause of accidental death in Brazil in children aged 0 to 4 years old is foreign body airway obstruction. Data from the Paraná Fire Department show that between 2010 and 2018, 2103 children who were victims of airway obstruction, aged 0 to 4 years old, were assisted in the state. It is in the age group from 0 to 3 years and 11 months old that children attend the Municipal Centers of Early Childhood Education - CMEI - and are under the responsibility of the teachers. The aim of the research was to evaluate the effect of a training course for the development of Heimlich Maneuver application skill, also known as abdominal thrusts, among the CMEI teachers of São Miguel do Iguaçu Municipality, western Paraná. This is a quantitative research, with quasi-experimental design, whose instrument for data collection was a questionnaire of thirty questions, consisting of closed-ended, trichotomous and multiple-choice questions with showcase and also Likert-like questions. The questionnaire was applied to teachers before and after an intervention, which constituted a theoretical and practical training on the Heimlich maneuver. Data were analyzed using descriptive statistics techniques. The analysis of the socio-demographic profile showed that 30.63% of the teachers are hired as interns. Teaching training courses do not offer first-aid subjects in their syllabus as it is not a requirement. A total of 57.66% of teachers reported having attended some first aid training course (p = 0.031). Most teachers (76.56%; p <0.0001) stated that they would be able to act partially in cases of airway obstruction in children. Before training, 70.27% of teachers demonstrated to recognize the importance of identifying the activity performed by the child when signs of airway obstruction appear. After training, the percentage increased to 86.49% (p = 0.004). Specifically about the airway clearance technique, after the intervention there were high percentages of hits. Regarding the number of spankings between the scapulae indicated for children under one year of age, there was an increase in the number of hits by 57.66% (p <0.001) compared to the pretest. The post-test results showed that the intervention measure was effective, considerably increasing the number of correct answers (with statistical significance), which points to the positive effect of the training offered to teachers. The study concluded that the intervention, which was a training on airway clearance with a theoretical and practical approach, improved teachers' knowledge regarding the technique of airway clearance in children.

Los datos del Departamento de Bomberos de Paraná muestran que entre 2010 y 2018, 2103 niños que fueron víctimas de obstrucción de las vías respiratorias, de 0 a 4 años, recibieron asistencia en el estado. Es en el grupo de edad de 0 a 3 años y 11 meses que los niños asisten a los Centros Municipales de Educación de la Primera Infancia (CMEI) y están bajo la responsabilidad de los maestros. El objetivo principal de la investigación fue evaluar el efecto de un curso de capacitación para el desarrollo de la habilidad de aplicación de maniobra de Heimlich, también conocida como maniobra de despeje de la vía aérea, entre los docentes del CMEI del municipio de São Miguel do Iguaçu, región al oeste del estado de Paraná. Esta es una investigación cuantitativa, con diseño cuasiexperimental, cuyo instrumento para la recopilación de datos fue un cuestionario de treinta preguntas, que consta de preguntas cerradas dicotómicas, tricotómicas, de opción múltiple con escaparate y también preguntas como Likert. El cuestionario se aplicó a los docentes antes y después de una intervención, lo que constituyó una capacitación teórica y práctica sobre la maniobra de compensación. Los datos se analizaron utilizando técnicas de estadística descriptiva. El análisis del perfil sociodemográfico mostró que el 30,63% de los docentes son contratados como pasantías. Los cursos de formación docente no ofrecen disciplina de primeros auxilios en su plan de estudios, ya que no es un requisito. Un total de 57.66% de los docentes informaron haber asistido a algún curso de capacitación en primeros auxilios (p = 0.031). La mayoría de los maestros (76.56%; p <0.0001) declararon que podrían actuar parcialmente en casos de obstrucción de las vías respiratorias en los niños. Antes del entrenamiento, el 70.27% de los maestros demostraron reconocer la importancia de identificar la actividad realizada por el niño cuando aparecen los signos de obstrucción de las vías respiratorias. Y después del entrenamiento, el porcentaje aumentó a 86.49% (p = 0.004). Específicamente con respecto a la técnica de limpieza de la vía aérea, después de la intervención hubo altos porcentajes de golpes. Con respecto al número de azotes entre las escápulas indicadas para niños menores de un año de edad, hubo un aumento en el número de golpes en un 57.66% (p <0.001) en comparación con la prueba preliminar. É na faixa etária de 0 até 3 anos e 11 meses que as crianças frequentam os Centros Municipais de Educação Infantil – CMEI – e ficam sob responsabilidade dos professores. O objetivo principal da pesquisa foi avaliar o efeito de um curso de capacitação para o desenvolvimento da habilidade de aplicação da Manobra de Heimlich, também conhecida como manobra de desobstrução de vias aéreas, entre os professores dos CMEI do Município de São Miguel do Iguaçu, região oeste do estado do Paraná. Trata-se de uma pesquisa quantitativa, com delineamento quase experimental, cujo instrumento para coleta de dados foi um questionário de trinta questões, composto por perguntas fechadas dicotômicas, tricotômicas, de múltipla escolha com mostruário e também questões tipo Likert. O questionário foi aplicado aos professores antes e depois de uma intervenção, que constituiu em um treinamento teórico e prático sobre a manobra de desobstrução. Os dados foram analisados por meio de técnicas de estatística descritiva. A análise do perfil sociodemográfico demonstrou que 30,63% dos professores são contratados em regime de estágio. Os cursos de formação dos professores do magistério não oferecem a disciplina de primeiros socorros em sua grade curricular, pois não é uma exigência. Um total de 57,66% dos professores afirmaram ter participado de algum curso de formação em primeiros socorros (p = 0,031). A maioria dos professores (76,56%; p < 0,0001) afirmou que saberia agir parcialmente em casos de obstrução das vias aéreas em crianças. Antes do treinamento, 70,27% dos professores demonstraram reconhecer a importância de identificar a atividade desempenhada pela criança quando do aparecimento dos sinais de obstrução das vias aéreas. E, após o treinamento, o percentual elevou-se para 86,49% (p = 0,004). Especificamente sobre a técnica de desobstrução das vias aéreas, após a intervenção houve elevados percentuais de acertos. Em se tratando do número de palmadas entre as escápulas indicadas para crianças menores de um ano de idade, houve um incremento no número de acertos em 57,66% (p < 0,001) em relação ao pré-teste. Os resultados obtidos no pós-teste mostraram que a medida de intervenção foi efetiva, aumentando consideravelmente o número de acertos (com significância estatística), o que aponta o efeito positivo do treinamento oferecido aos professores. The aim of the research was to evaluate the effect of a training course for the development of Heimlich Maneuver application skill, also known as abdominal thrusts, among the CMEI teachers of São Miguel do Iguaçu Municipality, western Paraná. This is a quantitative research, with quasi-experimental design, whose instrument for data collection was a questionnaire of thirty questions, consisting of closed-ended, trichotomous and multiple-choice questions with showcase and also Likert-like questions. The questionnaire was applied to teachers before and after an intervention, which constituted a theoretical and practical training on the Heimlich maneuver. Data were analyzed using descriptive statistics techniques. The analysis of the socio-demographic profile showed that 30.63% of the teachers are hired as interns. Teaching training courses do not offer first-aid subjects in their syllabus as it is not a requirement. A total of 57.66% of teachers reported having attended some first aid training course (p = 0.031). Most teachers (76.56%; p <0.0001) stated that they would be able to act partially in cases of airway obstruction in children. Before training, 70.27% of teachers demonstrated to recognize the importance of identifying the activity performed by the child when signs of airway obstruction appear. After training, the percentage increased to 86.49% (p = 0.004). Specifically about the airway clearance technique, after the intervention there were high percentages of hits. Regarding the number of spankings between the scapulae indicated for children under one year of age, there was an increase in the number of hits by 57.66% (p <0.001) compared to the pretest. The post-test results showed that the intervention measure was effective, considerably increasing the number of correct answers (with statistical significance), which points to the positive effect of the training offered to teachers. The study concluded that the intervention, which was a training on airway clearance with a theoretical and practical approach, improved teachers' knowledge regarding the technique of airway clearance in children.
