Resumo: The objective of this research was to carry out a Transcultural Adaptation of an instrument that assesses the barriers to medication adherence, the Adhesion Barriers Questionnaire (ABQ), written first in English. This adaptation occurs for Brazilian Portuguese, with necessary changes for elderly population understand the items. The methodology used followed the four-step procedure: translation of the original scale, back-translation of the translated scale, review by the Committee of Experts and pre-test with sampling of elderly patients. After translations and backtranslations, previous versions were analyzed by Committee of Experts, who made adjustments in the writing and understanding of items with the most appropriate language for a better understanding of the target population (elderly), taking into account characteristics of the population, such as literacy, visual and cognitive difficulties and self-specified format for the interview. The application of ABQ occurred in two stages (pre-test and pilot study); in both, the probing technique was performed. The difficulties encountered in the pre-teste were and modified for the pilot study. The sample oh the pre-teste and pilot study was 30 patients seen in each phase, aged 60 years or over, both sexes. These patients are followed up by the Primary Care System of the Unified Health System in a small port city in western Paraná. Of the interviewees, 58,33% were female, 68,33% had incomplete Elementary Education and 52,56% had Primary Essential Hypertension as their main diagnosis. For reliability analysis, Cronbach‟s and item-total. The most prevalent adherence barrier among participants referred to item 9 and the unintentional subscale was present in 43.33% of the responses presented. The Brazilian version of the ABQ questionnaire an important instrument adapted to the Brazilian linguistic and cultural context, with modifications aimed at the elderly population. Esta adaptação ocorreu para o contexto brasileiro com tradução para o português e alterações pertinentes à compreensão dos itens para a população idosa. A metodologia utilizada seguiu o procedimento de quatro etapas: tradução da escala original, retrotradução da escala traduzida, revisão por Comitê de Especialistas e realização do pré-teste com amostragem de pacientes idosos. Após as traduções e retrotraduções, as versões obtidas foram analisadas pelo Comitê de Especialistas, que realizou adequações na redação e compreensão dos itens, com linguagem mais adequada para compreensão pela população-alvo (idosos). Levando em consideração as características da população, como grau de letramento, além de dificuldade visual e cognitiva, escolheu-se a forma de aplicação por meio de entrevistas para a coleta de dados. A aplicação do ABQ ocorreu em duas etapas (pré-teste e estudo-piloto); em ambas foi realizada a técnica de sondagem. As dificuldades encontradas no pré-teste foram identificadas e modificadas para a realização do estudo-piloto. O tamanho da amostra do pré-teste e estudo-piloto foi de 30 pacientes atendidos em cada fase, com idade igual ou superior a 60 anos, ambos os sexos. Estes pacientes fazem acompanhamento pela Atenção Primária do Sistema Único de Saúde (SUS) em uma cidade de pequeno porte do Oeste do Paraná. Dos entrevistados, 58,33% eram do sexo feminino, 68,33% possuíam ensino fundamental incompleto e 52,56% possuíam Hipertensão Essencial Primária como diagnóstico principal. Para análise da confiabilidade, utilizou-se a correlação item-total e alfa de Cronbach. A barreira de adesão mais prevalente entre os participantes referiu-se ao item 9 e a subescala não intencional mostrou-se presente em 43,33% das respostas apresentadas. A versão brasileira do questionário ABQ é um importante instrumento adaptado ao contexto linguístico e cultural brasileiro, com modificações voltadas para a população idosa. Pesquisas futuras investigarão as propriedades psicométricas de validação deste questionário.
