Catheter – associated urinary tract infection: process indicators for analysis of prevention practices in critical patients

Resumo: Background and objective: Urinary tract infections have a significant impact on the patient's clinical evolution. In this way, the objective of this study was to analyze, through two process indicators. The conformities and nonconformities related to the practices of control and prevention of urinary tract infection related to delayed bladder catheterization in sectors with critical patients. Methods: Documental, observational and cross-sectional research of quantitative approach, performed in an Intensive Care Center and an Emergency unit of a public hospital in Curitiba. The data collection and analysis were based on process indicators elaborated and validated by Fernandez (2006). The indicators are evaluated accordingly when values between 73 – 87 % and 75 – 85 %, respectively. Results: For Indicators 1 and 2, the overall compliance rate was 68.8 % and 29.5 %, respectively. When the items that subsided the numerator of both indicators were registered, the indication for use of the device was more satisfactory in the Emergency (80.6 %) and the adequate fixation presented nonconformity in the two sectors evaluated (67.5 % in the Center for Intensive Care and 96.8 % in Emergency).Conclusion: In general, the indicators revealed nonconformities with regard to the prevention practices for urinary tract infection related to delayed bladder catheterization and address the need for the implementation of guidance strategies for care teams and continuous monitoring of catheter use. Catheters, Indwelling.Antecedentes y objetivo: El impacto infecciones del tracto urinario em gran medida de la evolución clínica del paciente, por lo que está dirigido en este estudio para analizar, a través de dos indicadores de procesos, Las conformidades y no conformidades relacionadas con las prácticas de control y prevención de infección del tracto urinario relacionado al cateterismo vesical de demora en sectores con pacientes críticos. Métodos: documental, observacional y transversal, conun enfoque cuantitativo, realizado en una unidad de cuidados intensivos y una unidad de emergencia de un hospital público de Curitiba. La recopilación y elanálisis de losdatos se basanen indicadores de proceso elaborados y validados por Fernández (2006). Los indicadores se evalúan enconsecuencia cuando los valores entre 73-87 % y 75-85 %, respectivamente. Resultados: Para los indicadores 1 y 2, el porcentaje global de cumplimiento em la Unidad de Cuidados Intensivos fue de 68,8 % y 29,5 %, respectivamente. Cuando se evaluólos elementos que so porta nel numerador de ambos indicadores, el registro de laindicación para el uso del dispositivo es el más adecuadoenlaemergencia (80,6 %) y adecuadafijacióninconformidad presentado ambos dos sectores (67,5 % enunidad de cuidados intensivos y el 96,8 % en estados de emergencia). Catéteres de Permanencia.Justificativa e Objetivos: as infecções do trato urinário (ITU) impactam de forma significativa na evolução clínica do paciente. Desse modo, objetivou-se analisar, por meio de Indicadores de Processo, as conformidades e não conformidades relacionadas às práticas de controle e prevenção de ITU relacionada ao cateterismo vesical de demora em setores com pacientes críticos. Métodos: pesquisa documental, observacional e transversal de abordagem quantitativa, realizada em um Centro de Terapia Intensiva (CTI) e uma unidade de Emergência de um hospital público de Curitiba. A coleta e análise dos dados foram baseadas em Indicadores de Processo elaborados e validados por Fernandez, Lacerda e Hallage (2006). Os Indicadores foram avaliados em conformidade quando seus valores estão entre 73 – 87 % (Indicador 1) e 75 – 85 % (Indicador 2). Resultados: para os Indicadores 1 e 2 o percentual geral de conformidade nos dois setores analisados foram de 68,8% e de 29,5%, respectivamente. Quando avaliados os itens que subsidiam os numeradores de ambos os Indicadores, o registro da indicação para o uso do dispositivo (Indicador 1) foi mais satisfatório no setor a Emergência (80,6%) e a fixação adequada (Indicador 2) apresentou não conformidade nos dois setores avaliados (67,5 % no CTI e 96,8 % na Emergência).
