Patient safety incidents occurred during nursing care

Resumo: Background and Objectives: Patient safety is a worldwide concern that has been highlighted in recent years and its study subsidizes preventive actions. The objective was to analyze patient safety incidents that occurred during nursing care. Methods: This was a retrospective, descriptive-exploratory documentary study carried out at a university hospital in southern Brazil. The documents related to patient safety incidents occurred during nursing care at the institution were classified and analyzed. Results: A total of 82 incidents were analyzed, of which 43 (52%) were classified as adverse events and 39 (48%) as incidents without harmful consequences. In 53 (64%) cases, the causes of the incidents occurred due to errors, 14 (17%) due to violations and 15 (18%) due to system failure. Incidents related to technical procedures were responsible for 67 (82%) occurrences, and adverse events occurred in 38 (57%) of them. Conclusion: Incidents causing both adverse events and those with no harmful consequences during care were identified, mostly caused by errors, followed by violations or system failures.

El objetivo fue analizar los incidentes de seguridad ocurridos con los pacientes durante el cuidado de enfermería. Métodos: investigación documental descriptiva-exploratoria retrospectiva realizada en un hospital universitario del sur de Brasil. Se clasificaron y analizaron los documentos relacionados con los incidentes de seguridad durante el cuidado de enfermería en la institución. Resultados: se analizaron 82 incidentes, de los cuales 43 (52%) fueron clasificados como eventos adversos y 39 (48%) como incidentes sin daño. En 53 (64%) casos las causas de los incidentes ocurrieron por errores, en 14 (17%) por violaciones y en 15 (18%) por falla en el sistema. Los incidentes relacionados con procedimientos técnicos contribuyeron con 67 (82%) ocurrencias, siendo que en 38 (57%) ocurrieron eventos adversos. Conclusión: identificados incidentes tanto con daño como sin daño durante el cuidado, causados en su mayoría por errores seguidos de violaciones o fallas en el sistema.Introdução: a segurança do paciente é preocupação mundial destacada nos últimos anos e seu estudo subsidia ações preventivas. Objetivo: analisar os incidentes de segurança ocorridos com os pacientes durante o cuidado de enfermagem. Método: pesquisa documental descritivo-exploratória retrospectiva realizada em um hospital universitário do sul do Brasil. Foram classificados e analisados os documentos relacionados a incidentes de segurança durante o cuidado de enfermagem na instituição. Resultados: foram analisados 82 incidentes, dos quais 43(52,5%) foram classificados como eventos adversos e 37(45,5%) como incidentes sem dano e 2% quase evento. Em 53(64%) casos as causas dos incidentes ocorreram por erros, em 14(17%) por violações e em 15(18%) por falha no sistema. Os incidentes relacionados a procedimentos técnicos contribuíram com 67(82%) das ocorrências, sendo que em 38(57%) ocorreram eventos adversos.
