Risks of infection in health care out of the hospital setting: an integrative review

Resumo: | Background and Objectives: healthcare-associated infections (HAIs) are common adverse events also in health care out of the hospital setting and generate increased costs and suffering by the individuals who acquire them. Considering the diversity and volume of outpatient services and the large number of users, this study aims to review the literature on HAI risks found in these facilities. Content: the present study is an integrative review of articles published between 2011 and 2016 in the following da-tabases: LILACS, MEDLINE / PubMed, and SciELO. The sample consisted of 13 articles. The risks of outbreaks due to airborne respiratory infections were demonstrated in the presence of multiresistant microorganisms, in unsafe injection practices and failures in the process of cleaning and sterilizing medical equipment in surgical, oncological and dialysis clinics. Patients at highest risk are the elderly, oncological and neurological patients, as well as incontinent ones and those with skin lesions and using catheters and drains. Conclusion: a specific legislation is required for all types of health services and it is necessary to increase the surveillance and regulation for the implementation of infection control programs that include indicators and education in the ser-vice. Ambulatory care facilities.Justificación y objetivos: las infecciones relacionadas con el cuidado de la salud también son comunes los efectos adversos en la atención sanitaria fuera del ámbito hospitalario y generan mayores costos y el sufrimiento de las personas para adquirirlos. Teniendo en cuenta la diversidad y volumen de los servicios de consulta externa y el gran número de usuarios, este estudio tiene como objetivo revisar la literatura el riesgo de infecciones hospitalarias en estos establecimientos. Contenido: este estudio es una revisión integradora de los artículos publicados entre 2011 y 2016 en LILACS, MEDLINE / PubMed, SciELO. La muestra está formada por 13 articulos. Se pone de relieve los riesgos de infecciones en los brotes de transmisión respiratorias en la presencia de gérmenes resistentes a múltiples fármacos en la inyección prácticas insegura y defectos en los procedimientos de limpieza y esterilización de equipo médico para clínicas quirúrgicas, oncología y diálisis. Los pacientes con mayor riesgo son los ancianos, oncológicos y neurológicos, lesiones en la piel y la incontinencia y el uso de sondas y drenajes. Conclusión: existe la necesidad de una legislación específica para todos los tipos de servicios de salud y aumento de la supervisión y el establecimiento de normas para la implantación de los programas de control de infecciones, incluyendo indicadores y educación en servicio. Instituciones de Atención AmbulatoriaJustificativa e Objetivos: as infecções relacionadas à assistência à saúde (IRAS) são eventos adversos comuns também nos serviços de saúde fora do contexto hospitalar e geram um aumento dos custos e sofrimento dos indivíduos que as adquirem. Considerando a diversidade e volume de serviços ambulatoriais e o grande número de usuários, este estudo tem como objetivo revisar na literatura os riscos de IRAS nesses estabelecimentos. Conteúdo: o presente estudo é uma revisão integrativa de artigos publicados entre 2011 e 2016 nas bases LILACS, MEDLINE/PubMed, SciELO. A amostra foi constituída de 13 artigos. Evidenciou-se os riscos da ocorrência de surtos por infecções de transmissão respiratória, na presença de microrganismos multirresistentes, nas práticas inseguras de injeção e em falhas nos processos de limpeza e esterilização de equipamentos médicos em clínicas cirúrgicas, oncológicas e de diálise. Os pacientes com maior risco são os idosos, oncológicos e neurológicos, os incontinentes e com lesões de pele e os que utilizam sondas e drenos. Conclusão: existe a necessidade de legislação específica para todos os tipos de serviços de saúde e aumentar a fiscalização e regramento para implantação de programas de controle de infecção que incluam indicadores e educação em serviço.
