Resumo: Keratoacanthoma is a rapidly growing, benign epithelial tumor that mainly affects areas exposed to the sunlight, such as the face, neck and forearm. It presents clinically as a solitary crateriform lesion with its interior filled by keratin, known to originate from the hair follicle. It usually targets light-skinned people, usually in the fifth decade of life, with a male predisposition three times higher. It resembles clinically and histologically with squamous cell carcinoma, which difficults and prioritizes its correct diagnosis. The clinical case presented describes an ulcerated lesion, located in vermilion of the lower lip, in an 83-year-old female patient. Significant changes were observed over a period of five months. Therefore, there was a change in clinical diagnosis to keratoacanthoma and the selected treatment was surgical excision. In the histopathological examination, was possible to observe an ulcerated area, hyperkeratosis, acanthosis, keratin pearls and continuity of the basal layer, confirming the diagnosis of keratoacanthoma. Thorough and detailed clinical examination is essential for the diagnosis of lower lip keratocanthoma. The patient has been under follow-up for 10 months with no sign of relapse. A rigorous clinical examination associated with a correct histopathological analysis are essential for the diagnosis of keratoacanthoma of the lower lip. The surgical ablation is performed by qualified professionals, being the treatment of choice

Apresenta-se clinicamente como uma lesão solitária crateriforme com seu interior preenchido por queratina, conhecida por originar-se do folículo piloso. Atinge preferencialmente pessoas de pele clara, geralmente na quinta década de vida, com uma predileção três vezes maior pelo sexo masculino. Assemelha-se clinicamente e histologicamente com o carcinoma espinocelular, o que dificulta e prioriza seu correto diagnóstico. O caso clínico apresentado, descreve uma lesão ulcerada, localizada no vermelhão do lábio inferior, em paciente do sexo feminino de 83 anos. Mudanças significativas foram observadas em um período de cinco meses. Portanto, houve a mudança do diagnóstico clínico para ceratoacantoma e o tratamento escolhido foi a excisão cirúrgica. No laudo histopatológico observou-se área ulcerada, hiperqueratose, acantose, pérolas de queratina e a continuidade da camada basal, confirmando o diagnóstico de ceratoacantoma. A paciente encontra-se em acompanhamento há 10 meses sem sinal de recidiva. Um exame clínico minucioso associado à uma correta análise histopatológica são essenciais para o diagnóstico do ceratoacantoma de lábio inferior.
