Avaliação da dor crônica e locomoção de cães com displasia coxofemoral submetidos à acupuntura

Resumo: The aim of this study was to evaluate chronic pain and gait analysis in dogs with hip dysplasia (HD) treated with acupuncture, carprofeno or plabebo. The study was a prospective, randomized, double-blind. A total of 47 dogs with lameness secondary to HD and 16 clinically normal dogs were enrolled. The HD dogs were randomized assigned to received acupuncture sessions (n=15), 4,4 mg/kg carprofen (n=16) or placebo (n=16) for 30 days. Owners completed the Canine Brief Pain Inventory (CBPI), Helsinki Chronic Pain Index (HCPI), Visual Analogue Scale for pain (VASpain) and a VAS for locomotion (VASloc) and dogs underwent pressure plate gait analysis two weeks (B1) and right before (B2) the first treatment, within two (W2), four (W4) and six (W6) weeks after treatment beginning. A veterinary also evaluated the locomotion by using a VASloc. Stance time, swing time, gait cycle and size, peak vertical forces, vertical impulse and vertical force distribution were recorded, and a symmetry index (SI) was calculated from each variable of the hindlimbs. There was no difference between treatments on owner's assessment. Pain scores decreased at the end and two weeks after the treatment was over for acupuncture dogs when CBPI and VASpain were used. The values of VASloc decreased for the acupuncture and carprofen dogs by the end of treatment. There were no differences on the gait analyses that indicate changes in lameness. We conclude that the gait analysis is not an effective tool to evaluate the resulting lameness from HD. Both acupuncture and carprofen decrease lameness by subjective evaluation and only acupuncture relieves pain associated with the HD and thus seems to be an option for improving the quality of life of animals with this condition

Para tal foram utilizados 47 cães com DCF e 16 cães saudáveis, em estudo clínico aleatório controlado e em dupla ocultação. Os animais com DCF foram tratados por 30 dias com acupuntura semanalmente (GA, n=15) ou 4,4 mg/kg de carprofeno por via oral uma vez ao dia (GT, n=16) ou placebo (GP, n=16). As avaliações foram realizadas duas semanas (B1) e imediatamente antes do tratamento (B2) e duas (S2), quatro (S4) e seis (S6) semanas após o início do tratamento. Os animais saudáveis foram avaliados somente uma vez. Os proprietários avaliaram a dor por meio do o Breve Inventário de Dor Canina (BIDC), o Indicador de Dor Crônica de Helsinque (IDCH) e a Escala Analógica Visual (EAVdor). Também se realizou a EAV para a locomoção (EAVloc). Um veterinário avaliou a claudicação por meio da EAVloc e uma Escala Numérica Descritiva (END). A avaliação cinética foi feita por obtenção dos índices de simetria (IS) das variáveis de tempo de apoio (TA), tempo de balanço (TB), tempo da passada (TP), comprimento da passada (CP), pico de força vertical (PFV), impulso vertical (IV) e distribuição do peso (DP) e da DP dos membros pélvicos. Não houve diferença significativa entre os tratamentos nas avaliações dos proprietários. Os escores de dor diminuíram ao final e duas semanas após o término do tratamento no grupo GA pelo BIDC e EAVdor. Os valores da EAVloc diminuíram em GA e GT ao final do tratamento. Na avaliação cinética não houve diferenças que indicassem alterações na claudicação. Conclui-se que a avaliação cinética não é uma ferramenta eficaz para avaliar a claudicação resultante da DCF.
