Análise comparativa dos acessos cirúrgicos para redução e fixação das fraturas do complexo zigomáticomaxilar

Introduction: the zygomaticmaxillary complex fractures are considered one of the most common facial trauma, and the most frequent causes are traffic accidents and assaults. Usually these fractures affect the junction points of the zygomatic bone to the adjacent bones. The recommended treatments are several and there are controversies regarding the choice of the best technique. Objective: compare, through 2 case reports of zygomatic orbital fractures, intra and extra-oral surgical access. Case Report: were analyzed the intraoral technique through the maxilar access and extraoral through the subciliary access to expose the infraorbital margin, regarding the treatment effectiveness. In the first case the patient have presented subconjunctival hemorrhage, fracture in the infraorbital rim, partial facial paralysis and limited mouth opening. The car accident have caused the zygomatic-maxillary fracture, mandibular and condyle fractures. Due to the physical aggression the patient, of the second case, have edema, periorbital ecchymosis, loss of projection of the zygomatic region and limited mouth opening, diagnosed with a fracture of the zygomatic-maxillary left side. Results: the two cases have shown that both techniques had satisfactory bone exposure for viewing and installing the plates and screws for fixation. The advantage of the intraoral technique is based on the absence of post-operative scar on the lower eye lid, while not allowing treatment of orbital floor, when committed; since the immediate post-operative swelling was more significtive using the extra-oral technique. Conclusion: both techniques were effective in the bone exposure when the fixation material was installed
