Tratamento de paciente irradiado com prótese implanto-retida, após oxigênioterapia hiberbárica

Around 50% of the patients with maligno neoplasia are submitted to radiotherapy which reduces the synthesis of new bone by osteocytes. In order to reduce the negative effects of the radiotherapy, the hyperbaric oxygen therapy has been used as part of the implant treatment protocol of irradiated patients. Therefore, the present study aimed to present a clinical report of an irradiated oncologic patient who underwent hyperbaric oxygen therapy in order to be rehabilitated with implant-supported prosthesis. A 60-year old man was admitted at Oral Oncology Center (FOA-UNESP) presenting a lesion on the mouth floor. After clinical evaluation, incisional biopsy and the histopathological exam, a grade II squamous cell carcinoma was diagnosed. The patient was subjected to surgery to remove the lesion and partial glossectomy. Afterwards, the radiotherapy (irradiation dose of 5040Gy), in the left/right cervicofacial area of the supraclavicular fossa was conducted. After 3 years of the surgery, the patient was submitted to hyperbaric oxygen therapy in a total of 30 sessions. Then, 4 implants were installed in patient's mandible, 2 on the premolar area (4.1ø x 13mm) and 2 on the anterior area (3.75ø x 13mm). Five months later, an upper conventional complete denture and lower full-arch implant-supported prosthesis were fabricated. The insertion of implants in the irradiated oncologic patient after hyperbaric oxygen therapy and subsequent rehabilitation with implant-supported prosthesis resulted in several benefits such as improvement on his chewing efficiency, swallowing and speech, less denture-trauma on the mucosa and improvement on his self-esteem
