Resumo: This study assessed the influence of bitewing radiographic examination of the treatment decision of caries lesions in primary and permanent teeth of children. Undergraduate students from Araçatuba Dental School, São Paulo State University (UNESP), regularly enrolled in the final semester (n = 74, participation rate 87%) took part in this study. Ten clinical cases, which were projected on screen and contained information about the patient's age and the lesion (ICDAS codes and activity) were selected. The same cases were presented again in random order, but including the bitewing radiograph of tooth in question. For each case, six possible treatments, which were subsequently dichotomized into invasive and non-invasive (n = 3 each) were given as possible options to the students. The association between treatment options was assessed by Chi-square test (p<0.05). Considering the 10 clinical cases together, it was observed that the majority of responses were related to invasive treatment options after the clinical examination alone (72.8%), and that bitewing radiographs significantly increased the percentage of responses related to invasive treatments to 78.7% (p = 0.011). Considering the cases separately, it was observed that the radiographic examination had a greater impact on lesions with ICDAS code 1 and 3, leading to an increase in the percentage of responses to invasive treatments in most of the cases. The impact of radiographic examination was lower for lesions with ICDAS code 4, while no influence of the radiographic images on the treatment decision was seen for lesions with ICDAS code 5. In conclusion, it was observed that radiographic examination had a direct impact on the treatment decision by the students, leading to a significant increase in the proportion of responses related to invasive treatments. The results indicate that the radiographic examination may be unnecessary in most cases in the treatment decision process

Participaram deste trabalho estudantes dos cursos de graduação em Odontologia da FOA-UNESP, regularmente matriculados no último semestre letivo (n=74, taxa de participação de 87%). Foram selecionados 10 casos clínicos, os quais foram projetados em tela e continham informações sobre a idade do paciente e sobre a lesão (códigos ICDAS e atividade). Os mesmos casos foram apresentados novamente em ordem aleatória, tendo-se incluído a radiografia interproximal do dente em questão ao lado da fotografia clínica. Em cada caso, foram oferecidas 6 possibilidades de tratamento, as quais foram posteriormente dicotomizadas em invasivas e não invasivas (n=3 cada). A associação entre as opções de tratamento foi avaliada pelo teste de Qui-quadrado (p<0,05). Considerando os 10 casos clínicos conjuntamente, observou-se que a maioria das respostas dos alunos esteve relacionada a opções de tratamentos invasivos quando do exame clínico (72,8%), sendo que o exame radiográfico elevou significativamente o percentual de respostas relacionadas a tratamentos invasivos para 78,7% (p=0,011). Considerando-se os casos isoladamente, observou-se que o exame radiográfico teve um maior impacto em lesões com código ICDAS 1 e 3, levando a um aumento do porcentual de respostas para tratamentos invasivos na maioria dos casos. O impacto do exame radiográfico foi menor para lesões com código ICDAS 4, não tendo sido verificado nenhuma influência deste exame sobre a decisão de tratamento para lesões com código ICDAS 5.
