Resumo: | Surgically assisted rapid maxillary expansion (SARME) is an effective treatment for maxillary transverse deficiency in adult patients. This study assessed the influence of two different osteotomy designs of the maxillary lateral wall on maxillary transverse changes, mandibular positioning and condyle fossa relationship after SARME. Thirty adult patients were divided into two groups according to the lateral osteotomy design: Group 1 (n = 16) underwent lateral osteotomy performed in a horizontal straight fashion and Group 2 (n = 14) underwent lateral osteotomy performed parallel to the occlusal plane with a step at the zygomatic buttress. Cone beam computed tomography scans were obtained before surgery (T1), immediately after expansion (T2) and six months after expansion (T3). The lateral osteotomy design had no bearing on the maxillomandibular changes after SARME. Both groups showed an increase in maxillary transverse measurements and clockwise rotation with lateral displacement of the mandible immediately after expansion. Maxillary changes showed to be stable six months after SARME, except for nasal floor width and tooth tipping which decreased at T3. Mandibular displacements tended to return close to their initial values at T3. No significant changes in condyle fossa relationship were observed at any time points. The design of lateral osteotomy of the maxilla did not affect the outcomes of SARME, which occurred by tipping of maxillary segments. Clockwise rotation and lateral displacement of the mandible are transient effects of SARME. |

Este estudo avaliou a influência de dois desenhos diferentes de osteotomia da parede lateral da maxila sobre as alterações transversais maxilares, de posicionamento mandibular e do relacionamento côndilo-fossa após ERMAC. Trinta pacientes adultos foram divididos em dois grupos de acordo com o desenho da osteotomia lateral. O grupo 1 (n = 16) foi submetido a osteotomia lateral realizada de forma horizontal e o grupo 2 (n = 14) submetido a osteotomia paralela ao plano oclusal com degrau no pilar zigomático. Tomografias computadorizadas de feixe cônico foram realizadas no pré-operatório (T1), após a expansão (T2) e seis meses após a expansão (T3). O desenho da osteotomia lateral não influenciou nas respostas maxilomandibulares após ERMAC. Nos dois grupos ocorreu aumento nas medidas transversais maxilares e rotação horária mandibular acompanhada de um deslocamento lateral imediatamente após a expansão. As alterações maxilares mostraram-se estáveis seis meses após a expansão, exceto a largura do assoalho nasal e inclinação dentária que apresentaram redução em T3. As alterações mandibulares tenderam a retornar aos valores próximos aos iniciais em T3. Não se observou alteração significativa no relacionamento côndilo-fossa em nenhum momento do estudo. O desenho da osteotomia lateral da maxila não influenciou nos resultados da ERMAC que ocorreu por meio de inclinação dos segmentos maxilares.
