Reabilitação estético-funcional com protése parcial fixa em zircônia monolítica

Resumo: Teeth loss generate several problems and require some prosthetic rehabilitation including possibilities as CD, RPD or FPD, associated or not to dental implants. Techniques were improved in FPD field and today is possible to fabricate monolithic structures reducing a lot of time of clinical care since no weld or additional procedures are required up to final crowns. So, besides save time, monolithic zirconium presents an easy technique as advantage once final format of crowns are sent directly by prosthetic laboratories. The patient presents more comfort during clinical care because repetitive sessions of provisional restoration removals and re-cementation are not needed. Maybe the unique negative factor is the higher cost when compared to conventional metal-ceramic crowns; however, considering number of clinical care and additional welds it is possible to consider not a high disadvantage. It must be highlighted that to maximum precision are required to this technique since no gaps could be present in the pillars of the FPD. So, it was aim of this study present a clinical case of a 55-years-old patient that looked for treatment with localized pain in the tooth #24. In the x-ray was diagnosticated needed for extraction. After discussion with the patient it was planned a monolithic zirconium FPD of 6 elements, including an anterior cantilever (tooth #22), with three pillars (teeth #23, #25 and #27) and two pontics (teeth #24 and #26). The treatment was realized and finalized, and the patient was very satisfied with the final result

Técnicas se modernizaram também a um ponto que hoje, dentro das próteses parciais fixas, existe a possibilidade de confecção de estruturas monolíticas que agilizam muito o atendimento clínico, visto que não necessitam de soldas ou procedimentos adicionais para a finalização do tratamento. Assim, além do tempo, seria ainda vantagem do uso de cerâmicas monolíticas a facilidade técnica, pois os elementos a serem substituídos já vem do laboratório no seu formato final. Além disso, principalmente o conforto ao paciente durante o atendimento, pois sessões repetitivas de remoção de provisórios e recimentação de provisórios não são recorrentes. Dentre suas desvantagens, seria o custo mais elevado quando comparado às metalocerâmicas convencionais que, considerando-se o número de sessões e o custo adicional por pontos de soldas, pode-se até considerar não ser um ponto tão negativo, por se diluir os gastos. Ressalta-se que deve ter precisão máxima para não haver distorção da peça protética gerando gaps nos dentes pilares. Assim, foi intuito deste trabalho apresentar um caso clínico de uma paciente de 55 anos de idade, IGS, que procurou por tratamento odontológico com dor localizada no elemento 24. Radiograficamente o mesmo se mostrou comprometido necessitando exodontia. Como o mesmo era elemento de PPF foi discutido o caso clínico com a paciente que optou pelo tratamento com PPF de 6 elementos, incluindo os dentes 22 (cantilever mesial), 23 (pilar), 24 (pôntico), 25 (pilar), 26 (pôntico) e 27 (pilar), do tipo zircônia monolítica com cobertura estética e maquiagem.
