Hiperplasia gengival associada ao uso de medicamentos anticonvulsivantes: revisão sistemática

Resumo: Gingival Hyperplasia associated with the use of medicines is an adverse effect of systemic drug treatment caused by three classes of drugs, anticonvulsants, immunosuppressants and calcium channel blockers. The objective of this study was to conduct a systematic review of the literature to ascertain the prevalence of Gingival Hyperplasia associated with the use of anticonvulsants and the main drug that leads to this clinical picture. A search was done on Pubmed database, using the words phenytoin, anticonvulsants, gingival overgrowth and gingival hyperplasia. After the collection of the articles, they were evaluated by two reviewers, who have selected the articles according to the following criteria: controlled clinical studies; prospective and retrospective studies and studies that evaluated the prevalence of Gingival Hyperplasia associated with the use of anticonvulsants. At the end were selected 8 articles, which showed prevalence gingival hyperplasia, ranging from 0 to 76%. It is concluded that one of the anticonvulsants used, phenytoin is the drug with a higher incidence of Gingival Hyperplasia associated with its use, varying their occurrence between 34.5% and 61.1% of the patients that use. However, studies are needed with a view to understanding the mechanism of action of drug induced gingival overgrowth of the, in addition to ways of preventing this increase, since gingival hyperplasia associated with the use of medicines remains a significant side effect, which is exacerbated by poor oral hygiene of patients with motor disabilities

O objetivo do presente trabalho foi realizar uma revisão sistemática de literatura para verificar a prevalência de hiperplasia gengival associada ao uso de anticonvulsivantes e qual o principal fármaco que leva a este quadro clínico. Foi realizada uma busca na base de dados Pubmed, utilizando as palavras phenytoin, anticonvulsants, gingival overgrowth e gingival hyperplasia. Após a coleta dos artigos, os mesmos foram avaliados por dois revisores, que selecionaram os artigos de acordo com os seguintes critérios: estudos clínicos controlados; estudos retrospectivos e prospectivos e estudos que avaliaram a prevalência da hiperplasia gengival associada ao uso de anticonvulsivantes. Ao final foram selecionados 8 artigos, que apresentaram prevalência da hiperplasia gengival, variando entre 0% a 76%. Conclui-se que dentre os anticonvulsivantes utilizados, a fenitoína é a droga com maior incidência de hiperplasia gengival associada ao seu uso, variando a sua ocorrência entre 34,5% e 61,1% dos pacientes que fazem uso.
