O uso de drogas anticoagulantes e antiplaquetárias orais em pacientes cirúrgico-ambulatoriais: revisão de literatura

Resumo: | Oral anticoagulants (AOC) and antiplatelet drugs are used for prevention of thromboembolic events such as pulmonary thromboembolism in patients with atrial fribrilação and / or heart valves, thrombosis and pulmonary embolism, it is considered vitamin K antagonists. The most known and widely used, warfarin sodium that acts by inhibiting or antagonizing factors II, VII, IX and X coagulation, vitamin K-dependent, and the anticoagulant proteins C and S, and aspirin that inhibits platelet aggregation. For dental patients indicated surgical procedures were asked to discontinue the use of anticoagulants, until the INR value remained ≥ 2.0 that in order to prevent bleeding during the surgery and postoperative. However, the interruption of the AOC can cause the formation of a blood clot leading to obstruction of the blood. Thus, it was doubtful whether the discontinuation of the drug to perform minor surgery, in which local hemostatic measures would be sufficient to stop blood flow. Thus the main purpose was to assess the risk of bleeding in patients using local AOC who undergo surgical procedures, through a literature review of relevant clinical trials published between 1990 and 2010, the Medline / Pubmed. In this literature review concluded, based on evidence from studies reported that the benefit of preventing thromboembolism outweighs the risk of bleeding, therefore, it is recommended to keep the dose of anticoagulant therapy unchanged for patients undergoing minor oral surgery and Implant Dentistry, working with therapeutic INR levels, and adopt local hemostatic measures - such as the use of antifibrinolytic agents, gels, compresses. Moreover, an accurate surgical technique should be performed in these patients. With respect to oral surgery more invasive with a risk of bleeding may be a need for treatment in conjunction with a medical team |

Os mais conhecidos e amplamente utilizados, a varfarina sódica que age inibindo ou antagonizando os fatores II, VII, IX e X da coagulação, dependentes da vitamina K e, das proteínas anticoagulantes C e S e, a aspirina que inibe a agregação plaquetária. Para pacientes odontológicos indicados a realização de procedimentos cirúrgicos eram solicitados a interromperem o uso dos anticoagulantes, até que o valor do INR se mantivesse ≥ que 2.0, de modo a evitar hemorragias no trans e pós-operatório. No entanto, a interrupção do AOC pode causar a formação de um coágulo sanguíneo, levando a obstrução das vias sanguíneas. Sendo assim, tornava-se duvidoso a interrupção do medicamento para a realização de cirurgias de pequeno porte, em que medidas hemostáticas locais seriam suficientes para interromper o fluxo sanguíneo. Deste modo, o objetivo da revisão foi avaliar o risco de hemorragia local em pacientes usuários de AOC que são submetidos à procedimentos cirúrgicos, por meio de um levantamento bibliográfico realizado em estudos clínicos relevantes publicados entre 1990 e 2010, pela base Medline/Pubmed. Nesta revisão de literatura concluiu - se, com base na evidência dos estudos relatados, que o benefício da prevenção de tromboembolismo supera o risco de hemorragia, assim sendo, recomenda-se manter a dose da terapia com anticoagulantes inalterada para pacientes submetidos à cirurgia oral menor e Implantodontia, trabalhando com níveis de INR terapêutico, e adotar medidas hemostáticas locais - como o de uso de antifibrinolíticos, gel, compressas.
