Análise do comportamento dos sintomas de ansiedade e depressão durante o primeiro mês de abstinência tabagística

Resumo: | INTRODUCTION: It is known that smoking causes a complex dependency mechanism. There are several factors that can hinder the smoking cessation, among these one can highlight the symptoms of anxiety and depression. Thus, it is essential to know the behavior of these symptoms to enrich the treatment and intervention to smoking. OBJECTIVE: To evaluate the behavior of anxiety and depression in individuals in the first month of tobacco smoke cessation. METHODS: The study was approved by the Ethics Committee and institutional research (245/2008) and included 14 individuals smokers of both sexes, from the smoking cessation program from FCTUNESP. An initial assessment was conducted for collection of anthropometric data, disease history, and level of dependency to nicotine through the Fargestrom test. Were excluded patients with a history of anxiety and depression diagnosed and individuals who did not use pharmacological therapy treatment proposed cessation (nicotine replacement therapy and Bupropion). To evaluate anxiety and depression was used the questionnaire HADS (Hospital Anxiety and Drepession Scale), applied in 5 periods: T0 (Basal-individual still smoking) T1 (first day of abstinence), T2 (seventh day), T3 (seventeenth day of abstinence) and T4 (29 day). These evaluation periods resulted from meetings held by handling the termination. These dates are defined by a calendar produced by the treatment team. ANOVA test was used for repeated measures or Friedman according to the normality of the data. RESULTS: The individuals evaluated 64.3% are male and 35.7% female. Showed an average age of 53 ± 12.56 years, BMI 26.95 ± 3, 702kg/cm2 average and nicotine dependence level of 6.385 ± 2.063 points, which features a high level of dependency to nicotine. In relation to levels of anxiety, comparing the baseline time (T0) with T1, there was a significant increase of 4.154 ± 2.60 points to 6.615 ± 3.798 (p < 0.001), to the other periods analyzed there was no significant difference. In relation to indices of depression, comparing the baseline period with T2, T3 and T4, there was a significant decrease of: 5,615±5,268 to 3±4,32, 3±3,76, 3,53±3,66, respectively (p = 0.007), for the period T1 there was no significant difference. CONCLUSION: There was an increase of anxiety during the first days of the abstinence, compared to the baseline period. In addition, reduction in signs of depression from the seventh day of abstinence. |

Existem vários fatores que podem dificultar a cessação do tabagismo, entre estes podem-se destacar os sintomas de ansiedade e depressão. Desta forma é imprescindível conhecer o comportamento desses sintomas para melhorar a forma de abordagem e o tratamento do tabagista. OBJETIVO: Avaliar o comportamento da ansiedade e depressão de tabagistas no primeiro mês de abstinência. MÉTODOS: O estudo foi aprovado pelo comitê de ética e pesquisa institucional (245/2008) e incluiu 14 tabagistas de ambos os sexos, provenientes do programa de cessação ao tabagismo da FCT-UNESP. Foi realizada avaliação inicial para coleta de dados antropométricos, histórico de doenças, e nível de dependência a nicotina por meio do teste Fargestrom. Foram excluídos pacientes com histórico de ansiedade e depressão diagnosticados e indivíduos que não utilizaram a terapia medicamentosa proposta pelo Programa de Cessação (terapia de reposição nicotínica e Bupropiona). Para avaliar a ansiedade e depressão foi utilizado o questionário Hospital Anxiety and Drepession Scale (HADS), aplicado em 5 períodos: T0 (Basal – indivíduo ainda fumando) T1, T2, T3 e T4 (primeiro, sétimo, décimo sétimo e vigésimo nono dia de abstinência, respectivamente). Estas datas são pré-estabelecidas por um calendário elaborado pela equipe do Programa de Cessação. Foi utilizado o teste de ANOVA para medidas repetidas ou Friedman de acordo com a normalidade dos dados. RESULTADOS: Dos indivíduos avaliados 64,3% são do sexo masculino e 35,7% feminino. Apresentaram uma média de idade de 53±12,56 anos, IMC médio de 26,95±3,702kg/cm2 e nível de dependência a nicotina de 6,385±2,063 pontos, o que caracteriza um nível de dependência á nicotina elevado. Em relação aos índices de ansiedade, comparando o T0 com T1, houve aumento significativo de 4,154±2,60 pontos para 6,615±3,798 (p<0,001), já para os outros períodos analisados não houve diferença significativa. Em relação aos índices de depressão, comparando o período basal com T2, T3 e T4, houve uma diminuição significante de: 5,615±5,268 para 3±4,32, 3±3,76, 3,53±3,66 respectivamente (p=0,007), já para o período T1 não houve diferença significativa. CONCLUSÃO: Houve aumento da ansiedade no primeiro dia de abstinência, comparado ao momento basal.
