Prevalência, severidade e impacto da incontinência urinaria específica da gestação em primigestas e secundigestas

Resumo: Introduction: Urinary incontinence (UI) is defined as the involuntary loss of urine. Pregnancy represents a cycle of great changes for the woman’s body and for the pelvic floor muscles, due to the weight of the fetus and the placenta, which can trigger specific pregnancy urinary incontinence (UI-EG). The literature does not bring us a concrete difference about urinary loss in primigravid and secundigravid women, therefore, the study addresses urinary loss in these different pregnant women. Objective: The objective of the present study was to analyze and compare the prevalence, severity and impact of IU-EG between primigravid and secundigravid women. Method: The study was carried out with pregnant women from 18 years old, at any gestational moment from 24 weeks, normoglycemic, urinary continents prior to the current pregnancy, without degenerative muscle diseases and who had never previously performed MAP training. The ICIQ-SF questionnaire was applied, which covers the frequency of urinary loss, volume and impact and the ISI questionnaire that says about the frequency and amount of urine loss. Results: In data analysis, there was a prevalence of UI-EG in the majority of pregnant women, with the secondary pregnancy group having the highest significant value. The most prevalent reason for loss was mixed urinary incontinence. About the final score of the questionnaires, there was no difference. However, the perception of the pregnant woman about the impact of UI on her life was higher in pregnant women who were secundigravid women. Conclusion: It was concluded thar there was a prevalence of UI-EG in most pregnant women regardless of parity, the severity was similar between the groups and the impact of UI-EG was more negative in pregnant women.

A gestação representa ciclo de grandes mudanças para o corpo da mulher e para os músculos do assoalho pélvico, devido ao peso do feto e da placenta, podendo desencadear a incontinência urinária especifica da gestação (IU-EG). A literatura não nos traz diferença concreta sobre a perda urinária em mulheres primigestas e secundigestas, diante disso, o estudo aborda a perda urinária nessas diferentes gestantes. Objetivo: O objetivo do presente estudo foi analisar e comparar a prevalência, severidade e impacto da IU-EG entre gestantes primigestas e secundigestas. Método: O estudo foi realizado com gestantes a partir de 18 anos, em qualquer momento gestacional a partir de 24 semanas, normoglicêmicas, continentes urinárias previas a atual gestação, sem doenças musculares degenerativas e que nunca tenham realizado previamente treinamento dos MAP. Foi aplicado o questionário ICIQ-SF que abrange a frequência da perda urinária, o volume e o impacto e o questionário ISI que diz a respeito da frequência e quantidade da perda de urina. Resultados: Em análise dos dados, houve prevalência de IU-EG na maioria das gestantes, sendo o grupo secundigestas com maior valor significativo. O motivo da perda mais prevalente foi a incontinência urinaria mista. Quanto ao escore final dos questionários não houve diferença. No entanto, a percepção da gestante sobre o impacto da IU em sua vida foi maior em gestantes secundigestas.
