Child-Caregiver-Pediatrician Communication: an Observational Study at Different Levels of Care

Resumo: Communication in pediatric settings presents many challenges, because of the presence of a triad (patient-caregiver-pediatrician) which requires specific skills in each step of an encounter. This study aimed to analyze and compare the communication in triads between the primary (health center) and tertiary (hospital) care levels. Twelve triads were observed, with the behaviors registered in specific protocols and recordings (audio and video). Two subsystems of interactional analysis were developed. In the first subsystem (purpose of communication content), the behavior of encouraging participation, encouraging interaction and communicating information was more observed in the health center. In the second subsystem (communication resources), facilitating and disruptive behaviors were also more recurrent in the primary care setting. Therefore, we conclude that in the health care center, consultations focused on patients, prevailed with greater incentive towards participation. In the hospital, the appointments focused mainly on the caregiver or the doctor.

Este estudio tuvo con objetivo analizar y comparar las comunicaciones de tríadas entre los niveles de atención primaria (centro de salud) y terciaria (hospital). Doce tríadas fueron observadas, siendo los comportamientos registrados en protocolos específicos y grabaciones (audio y vídeo). Dos subsistemas de análisis interacciónale fueron desarrollados. Se encontró que, en el primer subsistema (propósito de los contenidos comunicacionales), los comportamientos de incentivo a la participación y comunicar informaciones fueron más observados en el centro de salud. En el segundo subsistema (recursos de la comunicación), los comportamientos facilitadores y perturbadores también fueron más emitidos en la asistencia primaria. Se concluyó que en el centro de salud ha prevalecido una consulta centrada en el paciente, con mayor incentivo a su participación y que en el hospital, la consulta se centró principalmente en el acompañante o en el médico.A comunicação em contextos pediátricos apresenta muitos desafios, pois a presença de uma tríade (paciente-acompanhante-pediatra) exige competências específicas em cada etapa da consulta. Essa pesquisa visou analisar e comparar as comunicações de tríades entre os níveis de assistência primária (centro de saúde) e terciária (hospital). Doze tríades foram observadas, sendo os comportamentos registrados em protocolos específicos e gravações (áudio e vídeo). Desenvolveram-se dois subsistemas de análise interacional. Constatou-se que, no primeiro subsistema (finalidade dos conteúdos comunicacionais), os comportamentos de incentivo à participação, incentivo à interação e comunicar informações foram mais observados no centro de saúde. No segundo subsistema (recursos da comunicação), os comportamentos facilitadores e perturbadores também foram mais emitidos na assistência primária.
