Resumo: | Introduction: In breast reconstructive surgery, when in many of the patients there is lack of local tissues due to mastectomy, new materials able to cover silicone implants causing less inflammatory reaction and capsular contracture are still necessary. The most known material for this use is the acellular dermal matrix, which, most of the times, has a prohibitive cost. An alternative could be the use of polyester surgical meshes, which have been used for a long time in surgery. Objectives: Histological evaluation of the capsules formed by textured silicone implants non-covered and covered by polyester with porcine collagen sheet mesh. Material and method: Sixty Wistar rats were distributed into two groups with 30 animals each (without and with mesh coverage). Each group was then subdivided into two subgroups to be evaluated within 30 and 90 days post-surgery. Results: The amount of fibroblasts, neutrophils and macrophages was similar among all subgroups. There was a more exacerbated lymphocyte reaction on the 30 days subgroup (p=0.003) in the mesh group and no difference when the subgroups without and with the mesh where compared at 90 days. The foreign-body reaction, granulation tissue and neoangiogenesis were the same among all subgroups. Synovial metaplasia was more discreet in the 90 days in both groups (without mesh p=0.002 and with mesh p <0.001). There was no difference between the groups without and with mesh. The capsule thickness was significantly greater in the presence of mesh, in both subgroups (30 days p <0.001 and 90 days p<0.001). The amount of collagen types I and III was similar on both groups. Conclusions: The amount of fibroblasts, neutrophils and macrophages did not change with the presence of mesh. There was a greater lymphocyte reaction in the 30-day subgroup with mesh. The foreign body reaction, granulation tissue and neoangiogenesis were similar between the subgroups. Synovial metaplasia was more discreet at 90 days in both groups and the capsule thickness was greater with the presence of mesh, in both subgroups. The amount of collagen types I and III was similar on both groups. |
