Reparação óssea em cavidade cirúrgica de 8mm de diametro, preenchida com osso autógeno associado ao plasma rico em plaqueta e adesivo fibrínico : análise histológica em crânio de coelhos

Resumo: The modern Dentistry faces a challenge to correct bone defeats. The adequacy of architecture happen to the purpose to create conditions to insert osteointegrated implants, rehabilitating patients and returning, many times, an efficient chewed system, as well as maxillary reconstruction, repairing bone defeats with esthetic and functional purposes. In this way, this study evaluated bone repair in critic defeats done in rabbit skull, using autogenic bone graft associated to a fibrinic adhesive irrigated with epsilon-aminocaproic acid; autogenic bone graft associated to enriched plasma in platelets; autogenic bone graft with spontaneous clot and the Control Group without addition of graft material, only sanguine spontaneous clot by the own animal. They were used 32 white rabbits (New Zealand) male and adults, about 12 months old and weighing about three to four kilos. They were done bone defeats in the animal skull using a trefine drill of 10.0mm. After, these defeats were filled in materials and divided in groups. The Group I (Control) was filled in sanguine spontaneous clot by the own animal. In Group II, the defeats were filled in triturated cortical autogenic bone and gel of enriched plasma with platelets (PRP) and, in Group III, triturated cortical autogenic bone and fibrinic adhesive irrigated with EACA. In Group IV, triturated cortical autogenic bone with sanguine spontaneous clot by the own material. The animals were chosen by chance and divided in four groups. The period of sacrifice was 7, 15, 30 and 60 days, which were done the descriptive microscopical analyses with dyed blades in HE and Masson trichromic. The results showed that Control Group presented only a discrete bone formation at 60 day, near margins of the defeat. Groups II, III e IV showed similarity in the bone repair process; it is important, however, an observation about Group II (PRP), that showed a big volume in the period of 7, 14 and 30 days, and being equal of Group I and IV to the end of 60 days, that was observed bone mature lamelar tissue, with areas of bone remodelations. From these results, we concluded these materials used were similar, having no significant differences.

Estas adequações de arquitetura advêm do objetivo de criar condições para inserções de implantes osseointegrados, reabilitando pacientes e devolvendo, muitas vezes, um sistema mastigatório eficiente, bem como reconstruções maxilares, reparando defeitos ósseos com finalidades estéticas e funcionais. Desta maneira, o presente estudo avaliou a reparação óssea em defeitos críticos realizados em crânios de coelhos, utilizando enxerto ósseo autógeno associado ao adesivo fibrínico irrigado com ácido épsilon-aminocapróico; enxerto ósseo autógeno associado ao plasma enriquecido em plaquetas; enxerto ósseo autógeno com coágulo espontâneo e o Grupo Controle sem adição de material algum de enxertia, somente o coágulo sanguíneo espontâneo do próprio animal. Utilizaram-se 32 coelhos brancos (da raça Nova Zelândia) machos e adultos, com idade acima de 12 meses, e pesando entre três a quatro quilos. Foram realizados defeitos ósseos no crânio do animal utilizando uma broca trefina de 10,0mm. Posteriormente, estes defeitos foram preenchidos com materiais e divididos em grupos. O Grupo I (Controle) foi preenchido com coágulo sangüíneo espontâneo do próprio animal. No Grupo II, os defeitos foram preenchidos com osso autógeno cortical triturado e gel de plasma enriquecido em plaqueta (PRP) e, no Grupo III, osso autógeno cortical triturado e Adesivo fibrínico irrigado com EACA. No grupo IV, osso autógeno cortical triturado com coágulo sangüíneo espontâneo do próprio material. Os animais foram escolhidos ao acaso e divididos em quatro grupos. Os períodos de sacrifício foram de sete, 15, 30 e 60 dias, em que foi realizada a análise microscópica descritiva com as lâminas coradas em HE e tricrômico de Masson. Os resultados demonstraram que o Grupo Controle apresentou somente uma discreta formação óssea aos 60 dias, próximo às margens do defeito. Os grupos II, III e IV mostram similaridade no processo de reparo ósseo; porém, o Grupo II (PRP) apresentou um volume aumentado nos períodos de sete, 14 e 30 dias, e igualando-se aos grupos I e IV ao final de 60 dias, em que se observou tecido ósseo maduro lamelar, com áreas de remodelação óssea.
