Avaliação da estabilidade do avanço mandibular em osteotomias sagitais do ramo fixadas com miniplacas e parafusos monocorticais: estudo retrospectivo por meio de radiografias cefalométricas

Resumo: | Called dentofacial deformity as the skeletal growth defect of the facial bones, especially the jaws, resulting in changes in the proportion of those bones, positioning and inter-dental relationship, in addition to changes in speech, swallowing, breathing and aesthetics and harmony of the face. Among the osteotomies used for the correction of such deformities, the sagittal ramus osteotomy, first described by Trauner and Obwegeser, 1957, is surely the most widely used surgical technique in orthognathic surgery, especially for the treatment of both the excesses and deficiencies anterior posterior mandible. Due to its versatility, the scientific community has focused its attention hard to optimize their clinical results. Several factors have been linked to skeletal relapse events in the post-oerative procedures in advance and retreat by the technique of mandibular sagittal ramus osteotomy. Among these factors is discussed on the best type of rigid internal fixation for sagittal ramus osteotomy. Among the techniques employed for the fixation of sagittal ramus osteotomy, we can cite the use of one or two mini-plates of four or six holes and screws monocortical, or the use of hybrid technique with the combination of plates and screws monocortical bicortical screws, and using three bicortical screws, and each technique has particular characteristics regarding resistance to masticatory forces. This study aimed to evaluate the stability of mandibular advancement by means of lateral cephalometric analysis in patients undergoing orthognathic surgery performed through the technique of bilateral sagittal split osteotomy fixed with miniplates and screws system titanium 2.0mm. For that we have outlined the radiograph of ten patients, performed in the preoperative and postoperative early and late, this being the period and never more than six months, the technique of Burstone, with the help of the Dolphin Imaging 11.0. The values obtained from the mandibular length, measured from the gonial and condylar pogonion gnathion to the two times after surgery were evaluated by ANOVA. After statistical analysis, we concluded that the technique of fixation of sagittal ramus osteotomy, using titanium miniplates and screws monocortical, 2.0mm system, presents post-operative stability, and is considered a viable technique. |

Dentre as osteotomias utilizadas para a correção de tais deformidades, a osteotomia sagital do ramo mandibular, inicialmente descrita por Trauner e Obwegeser, 1957, é seguramente a técnica cirúrgica mais utilizada em cirurgia ortognática, especialmente para o tratamento tanto das deficiências, como dos excessos anteroposteriores da mandíbula. Devido à sua versatilidade, a comunidade científica tem arduamente direcionada a sua atenção na otimização de seus resultados clínicos. Diversos fatores têm sido relacionados aos eventos de recidiva esquelética no período pós-operatório em procedimentos de avanço e recuo mandibulares pela técnica da osteotomia sagital do ramo mandibular. Entre tais fatores é discutido qual o melhor tipo de fixação interna rígida para a osteotomia sagital do ramo mandibular. Dentre as técnicas empregadas para a fixação da osteotomia sagital do ramo mandibular, podemos citar a utilização de uma ou duas miniplacas de quatro ou seis furos e parafusos monocorticais, ou ainda a utilização de técnica hibrida com a combinação de placas com parafusos monocorticais e parafusos bicorticais, e a utilização de três parafusos bicorticais, sendo que cada técnica apresenta características particulares quanto à resistência aos esforços mastigatórios. O presente trabalho teve como objetivo avaliar a estabilidade do avanço mandibular, por meio de análise cefalométrica em norma lateral, em pacientes submetidos a cirurgias ortognáticas realizadas através da técnica da osteotomia sagital bilateral, fixados com sistema de miniplacas e parafusos de titânio de 2.0mm. Para isso, foram traçadas as telerradiografias de dez pacientes, realizadas no pré-operatório e pós-operatório imediato e tardio, sendo este o de maior período e nunca inferior a seis meses, pela técnica de Burstone, com o auxílio do programa Dolphin Imaging 11.0. Os valores obtidos do comprimento mandibular, mensurado do gônio ao pogônio e do côndilo ao gnátio nos dois tempos pós-operatórios foram avaliados pelo teste estatístico ANOVA. Após a análise estatística, foi possível concluir que a técnica de fixação da osteotomia sagital do ramo mandibular, por meio de miniplacas de titânio e parafusos monocorticais, do sistema 2.0 mm, apresenta estabilidade pós-operatória, sendo considerada uma técnica viável.Made available in DSpace on 2015-10-21T10:25:14Z (GMT).
