Obesidade sarcopênica e nível habitual de atividade física em indivíduos com diabetes mellitus e doença renal crônica em hemodiálise

The metabolic changes caused by type 2 diabetes mellitus, chronic kidney disease trigger mechanisms responsible for the development of sarcopenic obesity, considered as a risk factor for mortality, as well as reduced daily physical activity in this population, which may be associated with. The main of the present study is to investigate the prevalence of sarcopenic obesity in individuals with type 2 diabetes mellitus and chronic kidney disease undergoing hemodialysis and to investigate whether there is an association of sarcopenic obesity with levels of physical activity. It is a cross-sectional study that analyzed adult individuals of both sexes (men and women), diagnosed with DM2, submitted to hemodialysis treatment at two hemodialysis centers in the city of Bauru-SP. All patients from the referred hemodialysis centers were considered for the study, provided they met the inclusion criteria. The project was approved by the Research Ethics Committee of the “Universidade Sagrado Coração” and by the Teaching and Research Center of “FAMESP”. The information was collected through self-reported questions and through medical records. All the patients were evaluated for weight, height, body composition by electric bioimpedance and manual grip strength by the dynamometer. The patients were considered with sarcopenic obesity those with low skeletal muscle mass index and manual grip strength and percentage of body fat over 60%. The usual level of physical activity was evaluated by the Baecke questionnaire. The chi-square test evaluated possible associations between dependent and independent variables. The forward stepwise logistic regression model was constructed with the variables that presented associations statistical significance of up to 30% (p <0.30), using SPSS software version 13.0 with significance level defined in p0,05. The sample consisted of 84 individuals, 54.8% male, with a mean age of 63 ± 9.43 years, 25% of the patients presented sarcopenic obesity, and the level of physical activity was significantly associated with sarcopenic obesity (p <0.05). Sex, smoking and presence of coronary artery disease were associated with significance of p <0.30 with sarcopenic obesity. Sarcopenic obesity is independently associated with lower levels of physical activity (score <4.38) (OR = 3.33, 95% CI = 1.14 to 9.72, p = 0.02). It was concluded that 25% of individuals with type 2 diabetes mellitus and chronic kidney disease on hemodialysis had sarcopenic obesity and that this condition was associated with lower levels of habitual physical activity.
