Reparo alveolar de ratos tratados com bisfosfonato sob ação de terapia antimicrobiana e laser de baixa potência

Resumo: | Osteonecrosis of the jaws induced by bisphosphonates is considered a relevant complication when there is the necessity of performing surgical procedures in oral cavity, since prevention and treatment are not well established. In this way, the aim of the present study is to analyze the effect of low level laser therapy (LLLT) on socket healing of animals treated with BFs. A total of 64 male adult Wistar rats were distributed into 8 groups, with a total of 8 animals per group (4 animals – 14 days and 4 animals – 28 days), according to the treatment: Group 1 – Control, animals treated with endovenous (EV) 0.9% saline solution, Group 2 – treated with EV 0.9% saline solution under LLLT, Group 3 - treated with EV 0.9% saline solution under antibiotic therapy, Group 4 - treated with EV 0.9% saline solution under LLLT and antibiotic therapy, Group 5 – treated with EV zoledronic acid, Group 6 - treated with EV zoledronic acid under LLLT, Group 7 - treated with EV zoledronic acid and antibiotic therapy, and Group 8 - treated with EV zoledronic acid under LLLT and antibiotic therapy. 0.035 mg/Kg of the BF were administered every two weeks. After four weeks from the beginning of the BF administration, the animals were submitted to upper first molars extractions. Sodium benzilpenicilin were administered 3 days before and 4 days after the extractions. The sockets of the animals from Groups 2, 4, 6, and 8 were irradiated with the low level laser (AlGaAs - 808nm, CW, 30mW, Ø ~ 0,028 cm2) with a total dose of 64 J/cm2 in one point of irradiation, perpendicularly to the socket during one minute, with an interval of 24 hours for three days, beginning in the immediate post-operatory time. After 14 and 28 days from the teeth extractions, the animals were euthanized with lethal dose of anesthetics for the retrieval of the specimens to be prepared and analyzed considering morphological microscopy and immunohistochemistry. The use of LLLT alone did not prevent the installation of osteonecrosis and did not benefit in a significant way the repair process when the administration of zoledronic acid (ZL). The use of antibiotic alone, as well as its association with LLLT best possible repair decreasing the inflammatory process |

Deste modo, o presente estudo objetivou analisar a ação do laser de baixa potência (LTB) no reparo de alvéolos dentários em animais tratados com BF com e sem a associação de droga antimicrobiana. Um total de 64 ratos albinos Wistar machos adultos foram distribuídos em 8 grupos, com um total de 8 animais por grupo (sendo 4 animais para o período de 14 dias e 4 animais para o período de 28 dias) de acordo com o tratamento: Grupo 1 – Controle, animais tratados com soro fisiológico 0,9% via endovenosa (EV), Grupo 2 – tratados com soro fisiológico 0,9% EV e submetidos à LTB, Grupo 3 - tratados com soro fisiológico 0,9% EV e droga antimicrobiana, Grupo 4 - tratados com soro fisiológico 0,9% EV e submetidos à LTB e droga antimicrobiana, Grupo 5 - tratados com ácido zoledrônico EV, Grupo 6 - tratados com ácido zoledrônico EV submetidos à LTB, Grupo 7 – tratados com ácido zoledrônico EV droga antimicrobiana, e Grupos 8 - tratados com ácido zoledrônico EV submetidos à LTB e droga antimicrobiana. O BF foi administrado na dose de 0,035 mg/Kg a cada duas semanas. A medicação antimicrobiana benzilpenicilina sódica foi administrada 3 dias antes e 4 dias após as extrações dentárias. Após quatro semanas do início da administração do BF, foram realizadas as extrações dos dentes primeiros molares superiores dos animais de todos os grupos. Os alvéolos dentários dos animais dos Grupos 2, 4, 6 e 8 foram irradiados com laser de baixa potência (AlGaAs - 808nm, CW, 30mW, Ø ~ 0,028 cm2) na dose total de 64 J/cm2 em um único ponto de aplicação, perpendicularmente ao alvéolo por 1 minuto, com intervalos de 24 horas por três dias, iniciando no pós-operatório imediato. Após os períodos de 14 e 28 dias das extrações dentárias, os animais foram submetidos à eutanásia com dose letal de anestésico para a remoção dos espécimes. Posteriormente procedeu-se às análises microscópica morfológica e imuno-histoquímica. A utilização do LTB isoladamente não evitou a instalação da osteonecrose e não beneficiou de maneira importante o processo de reparo quando da administração do ácido zoledrônico (ZL).
