Avaliação das complicações mecânicas e biológicas nas próteses totais implanto suportadas: estudo retrospectivo

Resumo: Purpose: The purpose of this study was to evaluate the impact of biological and mechanical complications of fixed hybrid complete prostheses supported by implants in patients with at least one of the totally edentulous arches, and to determine their survival rates. Materials and Methods: The authors did a retrospective analysis of patients rehabilitated with this treatment which received fixed complete implant prostheses supported on maxilla, jaw and maxilla after reconstruction with bone graft from the iliac crest. All the prostheses are formed of a rigid metal frame with acrylic as a coating material. This study followed 20 patients (11 women and 09 men) for about of 64.6 months (minimum 01 and maximum 09 years), with a total of a hundred forty-five implants (65 in the jaw, 41on maxilla without bone graft and 39 in the maxilla reconstructed with iliac crest bone),being ten maxillaries prostheses and fourteen mandibular prostheses. Were evaluated the mechanical complications (fractures of the coating material, loosening and fractures of abutment screws and retention of the prostheses), biological complications (plaque index, depth and bleeding on probing, presence of keratinized tissue, suppuration, mucositis and peri-implantitis) and survival rate of prostheses and implants. Results and Conclusions: The most common mechanical complication was fracture of the coating material of the prostheses, no fracture of metal infrastructure has been found, as well as fractures of abutments and abutment screws. Fractures (2,94% maxillary and 6,25% jaw) and loosening (11,75% maxilla and 18,75% jaw) screws holding the prosthesis were found in both maxillary and mandibular prostheses. Were found presence of mucositis in the maxilla (5,88%) and mandible (10,93%), all the implants showed no suppuration, only one implant showed peri-implantitis in the maxillary and none in the jaw. The survival of the implants was 98,46% in the jaw, 97,56% in maxilla and 71,79% in the maxilla reconstructed with bone graft, the survival of prostheses was 100%

Materiais e Métodos: Os autores realizaram uma análise retrospectiva de pacientes reabilitados com esta modalidade de tratamento os quais receberam próteses totais fixas implanto suportadas em maxila, mandíbula e maxila após reconstrução óssea com enxerto ósseo de crista ilíaca. Todas as próteses se constituíam de uma estrutura metálica rígida tendo acrílico como material de revestimento. Foram acompanhados 20 pacientes (11 mulheres e 09 homens) por um tempo médio de 64.6 meses (mínimo de 01 e máximo de 09 anos), perfazendo um total de cento e quarenta e cinco implantes (65 na mandíbula, 41 na maxila sem enxerto ósseo e 39 na maxila reconstruída com osso de crista ilíaca), sendo dez próteses maxilares e quatorze próteses mandibulares. Foram avaliadas as complicações mecânicas (fraturas do material de revestimento, solturas e fraturas dos parafusos de abutment e de retenção das próteses), complicações biológicas (índice de placa, profundidade e sangramento à sondagem, presença de tecido queratinizado, supuração, mucosite e periimplantite) e o índice de sobrevivência das próteses e implantes. Resultados e Conclusões: A complicação mecânica mais comum foi a fratura do material de revestimento das próteses, nenhuma fratura de infra estrutura metálica foi encontrada, assim como fraturas de abutments e parafusos de abutment. Fraturas (2,94% maxila e 6,25% mandíbula) e afrouxamento (11,75% maxila e 18,75% mandíbula) de parafusos de retenção da prótese foram encontrados tanto em próteses maxilares quanto mandibulares. Foi encontrada presença de mucosite em maxila (5,88%) e mandíbula (10,93%), todos implantes apresentavam ausência de supuração, somente um implante apresentou periimplantite na maxila e nenhum em mandíbula.
