Reinternações no Hospital Estadual Bauru : ocorrência e possíveis causas

Resumo: Hospitalization besides being a traumatic procedure to the patient means higher social costs to the family and to society. Some patients may have to undergo repeated hospitalization that many times are related to the same health problem that caused the initial one. Non planned readmission to hospital means additional costs to health care expenditures be it a public or a private institution. It is conceivable that a significant number of these non planned hospital readmissions could be avoided if we had a better understanding of post discharge factors that may lead to readmission. The objective of this study was to evaluate readmission rate in a public hospital and to identify their causes. The work was conducted in the Bauru State Hospital, a public institution that provides health care to Bauru and other towns in the area. Data were collected through analysis of documents, hospital records and a personal interview with readmitted patients. A questionnaire was prepared with questions about any problem the patient had at home to take care of himself and to follow drug prescriptions. Results are based on data from 205 patients who had non planned readmission from January to July 2006. It was verified that almost all patients had spoken and or written instructions and drug prescription but that many of then failed to do so. Readmission rate was 3,1% overall, being about the same for both sexes. Our results also agree with other researches showing higher readmission rates for older patients (5,8%) and for children less than two years of age (8,2%).

Alguns pacientes sofrem com a necessidade de repetidas internações muitas vezes relacionadas com o problema de saúde que o levou anteriormente ao hospital. Para as instituições hospitalares essas reinternações não programadas representam um custo adicional que sobrecarrega o sistema financeiro da instituição, seja ela estatal ou particular. Um número significativo dessas internações/reinternações possivelmente poderiam ser evitadas com uma melhor compreensão das condições pós-alta que determinam a necessidade de reinternar. Procedeu-se o presente estudo com objetivo de avaliar o número de reinternações num hospital público e identificar as razões de sua ocorrência. O estudo teve como cenário o Hospital Estadual Bauru, uma instituição pública e estadual que atende a cidade de Bauru e outras cidades da região. Os dados foram coletados junto à instituição hospitalar, através de documentos, registros e entrevistas junto aos pacientes. Para tanto, foram utilizados como instrumentos para a coleta de dados um questionário, um roteiro de entrevista com pacientes reinternados, sendo os dados das entrevistas tabulados e analisados. Os dados dos pacientes foram complementados com outros obtidos dos relatórios administrativos. Os resultados são baseados em 205 pacientes que sofreram reinternação não programada no período de janeiro a julho de 2006. Mostram também que, apesar de os pacientes receberem recomendações e instruções sobre os medicamentos e cuidados a serem tomados em casa, existe pouca observância às mesmas. A porcentagem geral de reinternações no período estudado foi 3,1, sendo similar para o gênero masculino e feminino. Também confirma observações de outros autores sobre maior ocorrência de reinternações em pacientes idosos (5,8%) e em crianças com menos de dois anos de idade (8,2%).Made available in DSpace on 2015-09-14T09:17:47Z (GMT).
