Secular trends in malnutrition and obesity among children in S. Paulo city, Brazil (1974-1996)

Resumo: OBJECTIVE: Data from three household surveys undertaken in the city of S. Paulo, Brazil, from mid-80s to mid-90s allow to characterize and analyse secular trends in malnutrition and obesity among infants and children. METHODS: The three surveys included random population samples aged from zero to 59 months (1,008 children in the period of 1974-75; 1,016 in 1984-85 and 1,280 in 1995-96). The malnourished status was diagnosed when height-for-age and/or weight-for-height indices were below two standard deviation of the mean values expected according to the international growth reference. The obesity status was determined when weight-for-height indices fell two standard deviations above the reference. The study of the social distribution of malnutrition and obesity in each survey took into account tertiles of the per capita family income. For the study of the determinants of secular trends, hierarchical causal models, multivariate regression analyses and calculations analogous to the ones used to assess population attributable risks were applied. RESULTS/CONCLUSIONS: In the period of 22 years covered by the three surveys, child malnutrition was controlled in the city and became relatively rare even among the poorest families. The risk of obesity remained low and restricted to the richest families. Positive changes in distal (family income and maternal schooling) and intermediate determinants (sanitation, access to heath services and reproductive past history) of child nutritional status substantially explained part of the decline in the prevalence of malnutrition seen in the mid-80s to mid-90s.

OBJETIVO: Estimar a prevalência e a distribuição social da desnutrição e da obesidade na infância, estabelecer a tendência secular dessas enfermidades e analisar sua determinação, com base nos dados coletados por três inquéritos domiciliares, realizados na cidade de São Paulo em 1974/75, 1984/85 e 1995/96. MÉTODOS: Os três inquéritos estudaram amostras probabilísticas da população residente na cidade com idades entre zero e 59 meses (1.008 crianças em 1973/74; 1.016 em 1984/85 e 1.280 em 1995/96). O diagnóstico da desnutrição foi feito com base nos índices altura/idade e peso/altura adotando-se, como nível de corte, dois desvios-padrão da média esperada para idade e sexo segundo a referência internacional de crescimento. O diagnóstico da obesidade correspondeu a índices peso/altura, além de dois desvios-padrão da média esperada segundo a mesma referência. O estudo da distribuição social dos eventos de interesse levou em conta tercis da renda familiar per capita em cada um dos inquéritos. A estratégia analítica, para estudar os determinantes da tendência secular dos eventos de interesse, empregou modelos hierárquicos de causalidade, análises multivariadas de regressão e procedimentos análogos aos utilizados para calcular riscos atribuíveis populacionais. RESULTADOS/CONCLUSÕES: No período de 22 anos coberto pelos três inquéritos, a desnutrição na infância foi controlada na cidade de São Paulo, tornando-se relativamente rara mesmo entre as famílias mais pobres. O risco de obesidade permaneceu baixo e restrito às crianças pertencentes às famílias mais ricas. Mudanças positivas em determinantes distais (renda familiar e escolaridade materna) e intermediários (saneamento do meio, acesso a serviços de saúde e antecedentes reprodutivos) do estado nutricional justificaram parte substancial do declínio da desnutrição observado entre meados das décadas de 80 e 90.
